Brexpiprazole falls under Oral Drugs — the group Thyroid Extract interacts with.

Food and medications can interfere with thyroid hormone absorption. Some foods, particularly those high in fiber or containing certain minerals like calcium or iron, can bind to thyroid hormones and reduce their absorption in the digestive tract. Additionally, taking thyroid medications with certain drugs, like antacids or calcium supplements, can hinder the effectiveness of the thyroid medication, necessitating adjustments in dosage or timing. Prescription thyroid medications, such as Synthroid, are recommended to be taken on an empty stomach, at least 30 minutes before food and other medications, for optimal absorption.

Here the concern runs in one direction: your brexpiprazole (Rexulti) may slightly reduce how well your body absorbs the thyroid hormone in the extract. Like many oral tablets, brexpiprazole can sit in your stomach and gut alongside thyroid hormone, and thyroid hormone is famously picky about absorption. It really wants an empty stomach with nothing else nearby.

This is rated minor, so it is not dangerous, but it could leave the thyroid hormone working less reliably. A simple fix is spacing them apart, taking the thyroid product on an empty stomach and brexpiprazole separately. Since over-the-counter thyroid extract is unpredictable to begin with, please loop in your doctor or pharmacist before combining these.

About Brexpiprazole

What Is Brexpiprazole Used For?

Brand name: Rexulti

Brexpiprazole tablets are approved for two uses in adults: as an add-on (adjunctive) treatment alongside antidepressants for major depressive disorder (MDD), and as a treatment for schizophrenia. These uses apply to both the generic brexpiprazole tablets and Rexulti.

Rexulti has one additional approved use: treating agitation associated with dementia due to Alzheimer's disease in adults. Rexulti is also approved for schizophrenia in adolescents ages 13 and older. Note that Rexulti is not approved for dementia-related psychosis without agitation from Alzheimer's disease, and it is not meant to be used on an as-needed basis for Alzheimer's-related agitation.
