Theoretically, guggul might reduce the effects of CYP3A4 substrates.
In vitro research shows that guggul constituents known as guggulsterones can induce CYP3A4.

Here's the gist for Rexulti (brexpiprazole). Your liver uses an enzyme called CYP3A4 to break this medication down. Lab studies hint that guggulsterones, the active pieces of guggul, may speed that enzyme up. If that happens in real life, your body could clear brexpiprazole a little faster, which might soften how well it works and let symptoms like mood changes, anxiety, or psychosis creep back.

I want to be honest about the evidence here: this is theoretical, based only on test-tube and animal data, so we don't know how much (if at all) it matters in people. Food-style amounts are unlikely to be an issue, but concentrated supplements are less predictable. Please loop in your pharmacist or doctor before adding guggul so nobody changes your dose without a plan.

Guggul is a gum resin from the Commiphora wightii tree, long used in Ayurvedic medicine for cholesterol, joint, and skin problems. About Brexpiprazole

What Is Brexpiprazole Used For?

Brand name: Rexulti

Brexpiprazole tablets are approved for two uses in adults: as an add-on (adjunctive) treatment alongside antidepressants for major depressive disorder (MDD), and as a treatment for schizophrenia. These uses apply to both the generic brexpiprazole tablets and Rexulti.

Rexulti has one additional approved use: treating agitation associated with dementia due to Alzheimer's disease in adults. Rexulti is also approved for schizophrenia in adolescents ages 13 and older. Note that Rexulti is not approved for dementia-related psychosis without agitation from Alzheimer's disease, and it is not meant to be used on an as-needed basis for Alzheimer's-related agitation.
