Vitamin B6 and Mephenytoin

Here is the interaction we found between these two, with the evidence behind it.

Moderate

PHENYTOIN (Dilantin)

High doses of vitamin B6 may reduce the levels and clinical effects of phenytoin.
Preliminary clinical evidence suggests that vitamin B6 200 mg daily can reduce plasma levels of phenytoin, possibly by increasing metabolism. It is not known whether lower doses have any effect. Advise people taking phenytoin to avoid high doses of vitamin B6.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the thing to know about pairing high-dose vitamin B6 with mephenytoin (Mesantoin). Mephenytoin is a seizure medicine that works much like phenytoin, and some early reports suggest that large amounts of B6, around 200 mg a day, may speed up how your body breaks this drug down. If that happens, your blood levels could drift lower, and a seizure medication that's too low can mean less protection and a higher chance of breakthrough seizures.

I want to be honest that this is based on limited, anecdotal evidence, and we don't know if smaller doses matter at all. The B6 you get from food or a regular multivitamin is very unlikely to cause trouble. Before starting any high-dose B6 supplement, please run it by your pharmacist or doctor first.

From our pharmacist’s overview on the Mephenytoin page

“Anyone taking mephenytoin (Mesantoin) for seizures should read this page with genuine care rather than as background noise. The biggest cluster of interactions piles sedation on top of a drug that already slows the nervous system, and alcohol, whether beer or wine, sits at the serious end of that group.”

About Vitamin B6

What is Vitamin B6?

  • Morning sickness in pregnancy
  • Premenstrual syndrome (PMS)
  • Preventing or treating B6 deficiency
  • Nerve health and metabolism support
  • Nausea

Vitamin B6 (pyridoxine) is an essential water-soluble vitamin that your body needs for metabolism, brain function, and making red blood cells. It is best known for helping with pregnancy-related nausea and may help some people with PMS, but taking very high doses for long periods can cause nerve damage. Get most of your B6 from food, and check with a pharmacist or doctor before taking high-dose supplements.
