Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist: Fenfluramine (Fintepla) can have a calming, sedating side, and yerba mansa may add a little to that. The concern here is simply that combining the two could make your child feel more drowsy, sleepy, or tired than usual. Fenfluramine isn't a prodrug, so this isn't about one activating the other. It's just two things that may both nudge sedation in the same direction.

I'll be honest with you, this warning is theoretical, based on how these substances behave rather than real-world reports. Still, since fenfluramine treats serious seizure conditions, I'd loop in your doctor or pharmacist before adding yerba mansa, and watch for extra grogginess if you do.

From our pharmacist’s overview on the Fenfluamine page

“Alcohol is the danger I would flag first here. Fenfluamine (Fintepla) treats seizures in severe childhood epilepsies such as Dravet syndrome, and it is sedating by itself, so beer, wine, kava, and the other sedating ingredients on this page, 78 in all, can pile extra drowsiness on top, at worst to the point of suppressed breathing.”
