Moderate evidence
The evidence varies by strain and condition. L. acidophilus shows possibly effective evidence for irritable bowel syndrome (IBS), Helicobacter pylori infection, antibiotic-associated diarrhea, and bacterial vaginosis.
Bacillus subtilis is possibly effective for antibiotic-associated diarrhea. For most other conditions listed—allergic rhinitis, anxiety, asthma, athletic performance, and cognitive decline—the evidence we hold is insufficient to rate.

How safe is it?
Well-documented data
These probiotic strains are generally well tolerated in healthy adults. However, the safety data are limited, and caution is advised for people who are seriously ill, immunocompromised, or very young (especially premature infants)—there are rare documented cases of infection caused by these bacteria in those groups.
Uncommon side effects reported include mild gastrointestinal discomfort, bloating, flatulence, diarrhea, and in isolated cases, headache or rash. One ingredient, Bifidobacterium longum, is rated possibly safe in pregnancy and lactation; for L. acidophilus, no pregnancy/lactation data is on file.
For the three ingredients we could not check, safety data during pregnancy and breastfeeding aren't available either—talk with your doctor or pharmacist if you're pregnant or nursing.

Theoretically, taking Lactobacillus acidophilus with antibiotic drugs might decrease the effectiveness of L. acidophilus.
L. acidophilus preparations usually contain live and active organisms. Therefore, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and L. acidophilus preparations by at least two hours.
