The capsule itself contains hydroxypropylmethylcellulose as an inactive ingredient.

Strong evidence
The evidence we hold rates these ingredients' effectiveness as mostly insufficient to judge. Lactase is the one exception—it's rated effective for lactose intolerance.
Fennel seed is rated possibly effective for period cramps (dysmenorrhea). For the other uses listed in our data (papain for cancer, dental cavities, muscle soreness; bromelain for burns, hay fever, neuropathy pain; lipase and the other enzymes for digestive conditions), the research doesn't yet establish whether they work.
How safe is it?
Well-documented data
Papain is generally tolerated as a food but may irritate your digestive tract or trigger allergic reactions in sensitive people; avoid concentrated papain supplements and unripe papaya in pregnancy. Bromelain causes common side effects like diarrhea, gas, stomach upset, and headache; avoid it in pregnancy and breastfeeding.
Lipase has no reported side effects in adults but isn't well studied in pregnancy or while breastfeeding. Lactase is generally well tolerated and likely safe in pregnancy.
The protease enzymes may cause digestive upset or allergic reactions and should be avoided in pregnancy without medical supervision. Fennel seed is tolerated as a food, but concentrated supplements and the essential oil should be avoided in pregnancy and breastfeeding due to hormone-like effects; rare serious effects include seizures.
All these ingredients can cause allergic reactions in sensitive individuals, especially with occupational or prolonged exposure.

Bio.Revive Digestive + by Invivo contains several enzymes and botanicals, some of which interact with medications.
The most serious concern is papain, which may increase the effects and side effects of warfarin (a blood thinner). One patient previously stable on warfarin showed a dangerously high INR level after taking a papain-containing supplement.

Bromelain may increase bleeding risk if you take anticoagulants or antiplatelet drugs like aspirin or clopidogrel (Plavix); it can also theoretically raise tetracycline antibiotic levels, though human evidence is mixed. Fennel seed powder has several documented interactions: it may reduce how well birth control pills work, interfere with hormone replacement therapy, increase bleeding risk with blood thinners, lower ciprofloxacin (Cipro) levels by nearly half, weaken tamoxifen (a cancer drug), and theoretically raise levels of certain medications your liver processes through the CYP3A4 pathway.

Fennel seed powder
Anticoagulant/Antiplatelet Drugs
Theoretically, fennel might increase the risk of bleeding when used with antiplatelet or anticoagulant drugs.
Animal research suggests that fennel oil has antithrombotic and antiplatelet effects.
Ciprofloxacin (Cipro)
Theoretically, fennel might decrease the levels and clinical effects of ciprofloxacin.
Animal research shows that fennel reduces ciprofloxacin bioavailability by nearly 50%, possibly due to the metal cations such as calcium, iron, and magnesium contained in fennel. This study also found that fennel increased tissue distribution and slowed elimination of ciprofloxacin.
Contraceptive Drugs
Theoretically, taking large amounts of fennel might decrease the effects of contraceptive drugs due to competition for estrogen receptors.
Some constituents of fennel have estrogenic activity.
Cytochrome P450 3A4 (Cyp3A4) Substrates
Theoretically, fennel might increase levels of drugs metabolized by CYP3A4.
In vitro research suggests that fennel inhibits CYP3A4 enzyme activity. This effect has not been reported in humans.
Estrogens
Theoretically, taking large amounts of fennel might interfere with hormone replacement therapy due to competition for estrogen receptors.
Some constituents of fennel have estrogenic activity.
Tamoxifen (Nolvadex)
Theoretically, taking large amounts of fennel might decrease the antiestrogenic effect of tamoxifen.
Some constituents of fennel have estrogenic activity, which may interfere with the antiestrogenic activity of tamoxifen.
Bromelain
Anticoagulant/Antiplatelet Drugs
Bromelain may have antiplatelet effects and may increase the risk of bleeding if used with anticoagulant or antiplatelet drugs.
There is one case report of a patient experiencing minor bruising while taking bromelain with naproxen. Bromelain is thought to have antiplatelet activity. Whether this interaction is of concern with topical bromelain is unclear. Interference with coagulation of burn wounds has been reported in a patient receiving bromelain-based enzymatic debridement. However, observational research has found that topical bromelain debridement is not associated with increases or decreases in laboratory markers of coagulation when compared with surgical debridement.
Tetracycline Antibiotics
Theoretically, bromelain might increase levels of tetracycline antibiotics.
Laboratory research suggests that bromelain might increase the absorption of tetracycline antibiotics. However, a study in healthy adults reported no difference in tetracycline plasma levels when a 500 mg dose was taken with or without bromelain 80 mg.
Papain
Warfarin (Coumadin)
Theoretically, papain might increase the effects and side effects of warfarin.
In one case report, a patient previously stable on warfarin was found to have an international normalization ratio (INR) of 7.4, which was attributed to ingestion of a supplement containing papain from papaya extract.
