Not established
The evidence doesn't establish that these ingredients work for blood pressure support. Hawthorn carries ratings of insufficient evidence for angina, anxiety, arrhythmia, atherosclerosis, cardiovascular disease, and cognitive function.
Coleus is similarly unproven for angina, asthma, cancer, congestive heart failure, eczema, and dry eye. Olive leaf, linden flower, and mistletoe have no effectiveness ratings in our data — the evidence we hold simply hasn't rated them.
How safe is it?
Well-documented data
Olive leaf extract appears well tolerated in studies, though concentrated supplements are less studied than the food and oil form. The most common adverse effects reported are headache and stomach discomfort; one person in a trial developed skin problems.
Linden flower is generally well tolerated as a tea, but human safety data are limited; one case of allergic reaction to linden pollen and one case of skin irritation from a linden extract shampoo have been reported. Hawthorn seems well tolerated overall, though serious multiorgan allergic reactions with kidney failure have been reported rarely, and heart-related side effects like palpitations and circulation problems have occurred in a small number of patients in large trials.
Coleus is generally well tolerated, but gastrointestinal symptoms — especially diarrhea — occur frequently with oral use; nausea, vomiting, and constipation are also reported. When taken by other routes it may cause flushing, low blood pressure, or rapid heart rate.
Pregnancy and lactation safety data are insufficient for hawthorn and coleus — the safety data advises against use. Data aren't on file for olive leaf, mistletoe, and linden regarding pregnancy and breastfeeding.

Coleus Forskohlii Root Extract
Calcium Channel Blockers
Theoretically, combining coleus with calcium channel blockers might increase the coronary vasodilatory effects.
Forskolin, a constituent of coleus, and calcium channel blockers both cause coronary vasodilatory effects.
Nitrates
Theoretically, combining coleus with nitrates might increase the coronary vasodilatory effects.
Forskolin, a constituent of coleus, and nitrates both cause coronary vasodilatory effects.
Anticoagulant/Antiplatelet Drugs
Theoretically, concomitant use of coleus and anticoagulant or antiplatelet drugs might increase the risk of bruising and bleeding.
In vitro and animal research shows that forskolin, a constituent of coleus, can inhibit platelet aggregation and adhesion.
Antihypertensive Drugs
Theoretically, combining coleus with antihypertensive drugs might cause additive blood pressure lowering effects and increase the risk of hypotension.
Animal research shows that forskolin, a constituent of coleus, may lower blood pressure.
Cytochrome P450 2C9 (Cyp2C9) Substrates
Theoretically, taking coleus may affect drugs metabolized by CYP2C9 and increase the risk of adverse effects or reduce the effectiveness.
Research on the effect of coleus on CYP2C9 is conflicting. Some animal research shows that coleus extract can induce CYP2C9, while in vitro research shows that coleus can inhibit CYP2C9. Until more is known, advise patients that taking coleus might increase or decrease levels of drugs metabolized by CYP2C9.
Cytochrome P450 3A4 (Cyp3A4) Substrates
Theoretically, taking coleus might decrease serum levels of drugs metabolized by CYP3A4.
In vitro research shows that coleus can activate the nuclear receptor, pregnane X receptor (PXR), which results in increased expression of CYP3A4. Although the clinical significance of this is not known, use caution when considering concomitant use of coleus and other drugs affected by these enzymes.
Warfarin (Coumadin)
Theoretically, taking coleus may affect the metabolism of warfarin and increase the risk of adverse effects or reduce the effectiveness.
Some animal research shows that coleus extract can induce cytochrome P450 2C9 (CYP2C9), an enzyme that metabolizes warfarin. However, other in vitro research shows that coleus can inhibit CYP2C9. Theoretically, taking coleus with drugs metabolized by CYP2C9 might affect drug levels and the risk of adverse effects. Until more is known, advise patients that taking coleus might increase or decrease levels of warfarin.
Mistletoe Aerial Parts Extract
Antihypertensive Drugs
Theoretically, European mistletoe might increase the risk of hypotension when taken with antihypertensive drugs.
Animal research suggests that European mistletoe extracts have hypotensive effects.
Immunosuppressants
Theoretically, European mistletoe might decrease the effectiveness of immunosuppressants.
Clinical and in vitro studies suggest that European mistletoe might have immunostimulant effects.
Hawthorn Berry, Flowering Aerial Parts Extract
Nitrates
Theoretically, concomitant use might cause additive coronary vasodilatory effects.
Some evidence shows that hawthorn might lower blood pressure due to vasodilatory effects.
Phosphodiesterase-5 Inhibitors
Theoretically, concomitant use might result in additive vasodilation and hypotension.
Hawthorn might inhibit PDE-5 and cause vasodilation.
Anticoagulant/Antiplatelet Drugs
Theoretically, hawthorn may increase the risk of bleeding if used with anticoagulant or antiplatelet drugs.
In vitro and animal research shows that hawthorn can inhibit platelet aggregation. However, its effect in humans is unclear. One observational study shows that patients taking hawthorn shortly before undergoing coronary artery bypass graft (CABG) surgery or valve replacement surgery have a 10% incidence of postoperative bleeding, compared with 1% in those who never consumed hawthorn extract. However, clinical research shows that taking a specific preparation of dried hawthorn leaves and flowers (Crataesor, Soria Natural Lab) 800 mg three times daily for 15 days does not affect platelet aggregation or levels of thromboxane B2, the metabolite of thromboxane A2, in healthy humans.
Beta-Blockers
Theoretically, concomitant use might cause additive effects on blood pressure and heart rate.
Some evidence shows that hawthorn might lower blood pressure and heart rate.
Calcium Channel Blockers
Theoretically, concomitant use might cause additive coronary vasodilation and hypotensive effects.
Some evidence shows that hawthorn might lower blood pressure due to vasodilatory effects.
Digoxin (Lanoxin)
Theoretically, hawthorn might potentiate the effects and adverse effects of digoxin.
Hawthorn appears to improve cardiac output; however, hawthorn does not appear to affect digoxin pharmacokinetics. Case reports suggest that at least one species of hawthorn root extract (Crataegus mexicana) may produce adverse effects similar to digoxin and can cross-react with digoxin assays, leading to falsely elevated plasma digoxin levels.
Linden Flower with Bract Extract
Lithium
Theoretically, due to its potential diuretic effects, linden might increase the effects and adverse effects of lithium.
Linden is thought to have diuretic properties. The dose of lithium might need to be decreased.
