Boldo is well tolerated in food amounts but can cause serious liver damage when used in medicinal doses; it contains ascaridole, a known liver toxin, and cases of elevated liver enzymes and jaundice have been reported. Common side effects include nausea, vomiting, and abdominal pain.
Don't take this product during pregnancy — devil's claw may affect the uterus and boldo may stimulate it, and safety hasn't been established for either. Avoid it while breastfeeding because there isn't enough safety information for either ingredient.

Devil's claw may increase blood levels of several types of medications. It theoretically inhibits three major liver enzymes (CYP2C19, CYP2C9, and CYP3A4), which could raise levels of drugs those enzymes normally break down — all Moderate severity.
Devil's claw also carries a Moderate-severity risk with blood thinners like warfarin, with one case report of over-thinning. It may weaken heartburn medications (H2-blockers and proton pump inhibitors) and could increase levels of certain other drug types — these are Minor severity.
Boldo adds further Moderate-severity risks: it may boost the effect of blood thinners and antiplatelet drugs, raise lithium levels (potentially to harmful concentrations), and decrease the effectiveness of the immunosuppressant tacrolimus. Altogether, these interactions span 1,049 individual medications.

Devil's Claw Glycerite Liquid Extract
Cytochrome P450 2C19 (Cyp2C19) Substrates
Theoretically, devil's claw might increase levels of drugs metabolized by CYP2C19.
In vitro research shows that devil's claw might inhibit CYP2C19, although this has not been reported in humans.
Cytochrome P450 2C9 (Cyp2C9) Substrates
Theoretically, devil's claw might increase levels of drugs metabolized by CYP2C9.
In vitro research shows that devil's claw might inhibit CYP2C9, although this has not been reported in humans.
Cytochrome P450 3A4 (Cyp3A4) Substrates
Theoretically, devil's claw might increase levels of drugs metabolized by CYP3A4.
In vitro research shows that devil's claw might inhibit CYP3A4, although this has not been reported in humans.
Warfarin (Coumadin)
Theoretically, Devil's claw might increase the activity of warfarin.
In one case report, purpura occurred in a patient taking warfarin and devil's claw concurrently. This might indicate over-anticoagulation. It is unclear if this was due to Devil's claw or other contributing factors.
H2-Blockers
Theoretically, devil's claw might decrease the effectiveness of H2-blockers.
Devil's claw has been reported to increase stomach acid, which might interfere with the effects of H2-blockers.
P-Glycoprotein Substrates
Theoretically, devil's claw might increase levels of P-glycoprotein substrates.
In vitro research shows that devil's claw inhibits P-glycoprotein, which transports many drugs out of cells. This might increase intracellular levels of P-glycoprotein substrates, although it is unclear if this effect would be clinically important.
Proton Pump Inhibitors (Ppis)
Theoretically, devil's claw might decrease the effectiveness of PPIs.
Devil's claw has been reported to increase stomach acid, which might interfere with the effects of PPIs.
Boldo Glycerite Liquid Extract
Anticoagulant/Antiplatelet Drugs
Theoretically, taking boldo with anticoagulant/antiplatelet drugs might increase the risk of bleeding.
Animal and in vitro research shows that boldine, a constituent of boldo, has antiplatelet activity. In one case report, an adult taking a combination of boldo and fenugreek with warfarin experienced an increase in international normalized ratio (INR); however, it is unclear if this effect was due to boldo, fenugreek, the combination, or another factor.
Hepatotoxic Drugs
Theoretically, taking boldo with hepatotoxic drugs might increase the risk of hepatic injury and disease.
Boldo leaf contains ascaridole, a known liver toxin. Many cases of hepatotoxicity, including elevated liver transaminase levels and jaundice, have been reported in patients taking boldo.
Lithium
Theoretically, taking boldo with lithium might increase the levels and clinical effects of lithium.
Boldo is believed to have diuretic effects. Theoretically, these diuretic effects might reduce the excretion of lithium. The dose of lithium might need to be decreased.
Tacrolimus (Prograf)
Taking boldo with tacrolimus may decrease the levels and clinical effects of tacrolimus, potentially increasing the risk of transplant rejection.
In one case report, a patient with a long-term history of stable tacrolimus levels developed subtherapeutic levels after taking boldo 300 mg twice daily orally for several weeks. Tacrolimus levels returned to normal after discontinuing boldo. However, the mechanism of this interaction is unclear.
Warfarin (Coumadin)
A combination of boldo and fenugreek was thought to be associated with an increased international normalized ratio (INR) in a female on warfarin. It is not clear if boldo, fenugreek, or the combination played a role in this interaction. Therefore, boldo may have additive effects with warfarin.
