Manganese in normal dietary amounts and the amounts found in prenatal vitamins are safe. High-dose supplements, however, carry real risks.

At high doses, manganese is generally well tolerated at first, but serious side effects—rare but severe—can include manganese poisoning (manganese intoxication) and Parkinson-like neurological symptoms. One case report documented irreversible Parkinson disease possibly linked to 100 mg daily for 2 to 4 years.

A severe case in a teenager exposed to high manganese levels led to life-threatening neurological symptoms, incomplete cirrhosis, and fatty liver disease requiring chelation therapy and plasma exchange to recover. For pregnancy and lactation, the safety data shows mixed ratings—breastfeeding carries ratings from likely safe to likely unsafe, so talk with your pharmacist or doctor for personalized guidance before using this product during lactation.

Theoretically, the risk for manganese toxicity might increase when taken with antipsychotic drugs.
Hallucinations and behavioral changes have been reported in a patient with liver disease who was taking haloperidol and manganese. Researchers speculate that taking manganese along with haloperidol, phenothiazine-derivatives, or other antipsychotic medications might increase the risk of manganese toxicity in some patients.
