HelloPharmacist Scorecard of NSK-SD 100 mg by Pure Encapsulations
Our pharmacy team’s full take, with four database checks built into the cards below — a summary of what is known, not a grade of the product itself.
What’s inside
Full disclosure
NSK-SD 100 mg is a single-ingredient supplement: nattokinase (NSK-SD), a protein-digesting enzyme derived from natto, a traditional fermented soybean food. The capsule also contains inactive ingredients—hypoallergenic plant fiber, a vegetarian capsule shell, and ascorbyl palmitate (a form of vitamin C used as a preservative).
Does it work?
Not established
The evidence for nattokinase isn't established in the data we hold. The ingredient carries ratings of insufficient reliable evidence for angina, atherosclerosis, cancer, cardiovascular disease, and chronic fatigue syndrome.
That means the research so far hasn't been robust enough to say whether it works for these purposes.
How safe is it?
Well-documented data
Nattokinase is generally well tolerated short-term, but its blood-thinning properties mean you need to be cautious if you have any bleeding risk. Rare cases of bleeding—including one serious brain bleed—have been reported.
It can also trigger hypersensitivity reactions (including anaphylaxis) in people sensitive to natto or its components, with symptoms appearing within 2 hours of taking it. There isn't enough safety data to use this product during pregnancy, so the data advises against it.
Safety while breastfeeding is unknown, so it's best avoided then too.
Meds to double-check
Moderate interaction found
Double-check this product if you take blood thinners or antiplatelet drugs (warfarin, dabigatran, apixaban, aspirin, clopidogrel, ticagrelor, and others) — Moderate risk of increased bleeding. Also check if you're on blood pressure medications — nattokinase may lower your pressure further.
The bottom line
Scorecard at a glanceFully disclosed formula with no established evidence rating for its marketed use. Moderate medication interactions have been identified, and safety information is well characterized.
NSK-SD is a blood-thinning enzyme supplement. If you take any blood thinner, antiplatelet drug (like aspirin), or blood pressure medication, check with your pharmacist before starting—the interaction risk is real.
If you're pregnant, breastfeeding, or have a bleeding disorder, talk to your doctor first.

NSK-SD 100 mg by Pure Encapsulations Drug Interactions
HelloPharmacist Interaction Report
NSK-SD 100 mg from Pure Encapsulations contains nattokinase, an enzyme that thins the blood — and it has documented interactions with two major drug categories.
The most serious concern is with blood thinners and antiplatelet drugs (like warfarin, dabigatran, and aspirin). Nattokinase itself has blood-thinning and clot-dissolving effects, and there's a case report of brain bleeding in someone who combined it with low-dose aspirin.
This is a Moderate severity interaction.
Read the full breakdown — every affected drug type, severity by severity
It can also lower blood pressure when paired with blood pressure medications (antihypertensives), which is also Moderate severity. Some research suggests nattokinase may reduce both systolic and diastolic pressure on its own.
Altogether, these interactions span 287 individual medications. If you take any prescription blood thinner, antiplatelet drug, or blood pressure medication, run your exact drug list through the checker below before starting this product.
