Tribulus is generally well tolerated in the short term for healthy adults, but long-term safety data and product quality information are limited. Serious side effects are rare but have been reported — these include liver and kidney injury, seizures, and priapism (a persistent, painful erection that impairs sexual function).

More common mild side effects are abdominal pain, cramping, nausea, vomiting, diarrhea, and constipation, though in one study these occurred at similar rates in people taking tribulus and those receiving placebo. Do not take tribulus during pregnancy — it may affect hormones and safety is not established.

Taking tribulus with antidiabetes drugs might increase the risk of hypoglycemia.
