Ultimate Enzymes contains 20 active ingredients, most of them digestive enzymes that help break down different food components. Amylase breaks down starches; papain and bromelain digest proteins; lipase breaks down fats; lactase helps with lactose from dairy.
Others like cellulase, invertase, and glucoamylase target fiber and sugars. The product also contains nattokinase and serrapeptase — enzymes with blood-thinning effects — plus protease, a general protein-digesting enzyme.
The capsules themselves are made from cellulose, gum acacia, rice husk, and a mineral blend listed as inactive ingredients.

Strong evidence
The evidence on these enzymes is largely inconclusive. Lactase is effective for lactose intolerance — that's the one clear win.
For everything else in this formula — papain, bromelain, lipase, nattokinase, serrapeptase, and protease — the research either hasn't been done or isn't reliable enough for us to rate them. That includes claims about digestion, energy, inflammation, heart health, and muscle soreness.

How safe is it?
Well-documented data
Most of these enzymes are generally well tolerated at normal doses. Papain, bromelain, and serrapeptase can cause digestive upset, nausea, or allergic reactions in some people — watch for itching, hives, or breathing changes.
Nattokinase carries a rare but serious risk of bleeding. Lipase and phytase are considered low-risk, though long-term data are sparse.
Pregnancy and breastfeeding: papain, bromelain, nattokinase, and serrapeptase should be avoided during pregnancy due to insufficient safety data. Lactase is likely safe in pregnancy.
Lipase, phytase, and protease have limited pregnancy data — talk to your doctor. While breastfeeding, the safety picture is unclear for most of these; check with your healthcare provider before taking them.

Nattokinase
Anticoagulant/Antiplatelet Drugs
Nattokinase might increase the risk of bleeding when used with anticoagulant/antiplatelet drugs.
Nattokinase has thrombolytic and anticoagulant effects. There is a case report of intracranial hemorrhage in a patient who added nattokinase to aspirin therapy.
Antihypertensive Drugs
Theoretically, nattokinase might increase the risk of hypotension when used with antihypertensive drugs.
Some clinical research suggests that nattokinase may reduce systolic and diastolic blood pressure.
Bromelain
Anticoagulant/Antiplatelet Drugs
Bromelain may have antiplatelet effects and may increase the risk of bleeding if used with anticoagulant or antiplatelet drugs.
There is one case report of a patient experiencing minor bruising while taking bromelain with naproxen. Bromelain is thought to have antiplatelet activity. Whether this interaction is of concern with topical bromelain is unclear. Interference with coagulation of burn wounds has been reported in a patient receiving bromelain-based enzymatic debridement. However, observational research has found that topical bromelain debridement is not associated with increases or decreases in laboratory markers of coagulation when compared with surgical debridement.
Tetracycline Antibiotics
Theoretically, bromelain might increase levels of tetracycline antibiotics.
Laboratory research suggests that bromelain might increase the absorption of tetracycline antibiotics. However, a study in healthy adults reported no difference in tetracycline plasma levels when a 500 mg dose was taken with or without bromelain 80 mg.
Serrapeptase
Anticoagulant/Antiplatelet Drugs
Theoretically, taking serrapeptase with drugs that have antiplatelet or anticoagulant effects might increase the risk of bruising and bleeding.
In vitro research shows that serrapeptase has fibrinolytic activity.
Papain
Warfarin (Coumadin)
Theoretically, papain might increase the effects and side effects of warfarin.
In one case report, a patient previously stable on warfarin was found to have an international normalization ratio (INR) of 7.4, which was attributed to ingestion of a supplement containing papain from papaya extract.
