UTI Clear contains 5 active ingredients. D-mannose is a simple sugar that your body uses differently than regular glucose.
Cranberry extract is a concentrated form of the fruit long used for urinary health. Berberine is a plant alkaloid from herbs like goldenseal and barberry, used to support metabolic health.
Uva ursi (bearberry leaf) is an herbal remedy traditionally used for the urinary tract. The product also includes a probiotic ingredient — we hold no data on which strain or how much is in the formula.
The capsules are made with cellulose and diatomaceous earth as inactive ingredients.
Does it work?
Moderate evidence
The evidence for this product's ingredients is mixed. D-mannose is rated possibly ineffective for urinary tract infections — the research doesn't support it as a treatment.
Cranberry is rated possibly effective for UTIs, so there's some clinical support, though results vary. Uva ursi is also rated possibly ineffective for UTIs.
Berberine shows possibly effective ratings for several other conditions — high cholesterol, PCOS, Helicobacter pylori infection, high blood pressure, and diabetes — but those are outside this product's intended use. How safe is it?
Well-documented data
D-mannose is generally well tolerated in the short term, but long-term safety data are limited. The most common side effects are bloating, diarrhea, and nausea — especially at higher doses.
Safety data during pregnancy are not available; the facts advise against use unless your doctor says otherwise. Safety while breastfeeding is not well studied, so caution is advised.
Cranberry is generally well tolerated as food or supplements in normal amounts, though concentrated supplement doses haven't been well studied. Diarrhea and gastrointestinal discomfort are the most common effects.
Cranberry is rated likely safe in pregnancy and lactation. Berberine should be avoided during pregnancy — it may cross the placenta and has been linked to harm in newborns — and avoided while breastfeeding because it passes into breast milk.
Common side effects are abdominal pain, constipation, diarrhea, nausea, and vomiting; it may also cause dizziness, drowsiness, and headache. Uva ursi is generally well tolerated at low doses short-term but can be toxic at high doses or with prolonged use.
Avoid it during pregnancy as it may stimulate the uterus and lacks safety data. Insufficient safety information exists for breastfeeding, so it should be avoided.
Common side effects include nausea, vomiting, diarrhea, and stomach upset.

The most serious concern is berberine, which can significantly raise levels of cyclosporine (an immunosuppressant drug), increasing the risk of toxicity — this is a Major interaction. If you take cyclosporine, do not use this product without talking to your doctor first.

Berberine also poses Moderate risks with several other drug types: it may add to the blood-pressure-lowering effects of blood pressure medications (antihypertensive drugs), increase the sedating effects of sedatives and sleep aids (CNS depressants), raise levels of blood thinners and antiplatelet drugs and boost bleeding risk, and lower blood sugar further in people taking diabetes medications. It can also increase levels of drugs metabolized by three liver enzymes (CYP3A4, CYP2D6, and CYP2C9 substrates).
Cranberry poses Moderate interactions with cholesterol-lowering drugs like atorvastatin (Lipitor), the blood pressure medication nifedipine (Procardia), and blood thinners like warfarin (Coumadin), as well as with drugs metabolized by CYP3A4. It carries a Minor interaction with nonsteroidal anti-inflammatory pain relievers (NSAIDs) like diclofenac and with other CYP2C9 substrates.
Uva ursi theoretically interacts with several drug types at Moderate severity: medications metabolized by CYP2C19 and CYP3A4, drugs processed by glucuronidation, lithium (a mood stabilizer), and urinary acidifying agents. It also has a Minor interaction with P-glycoprotein substrates.

Berberine
Cyclosporine (Neoral, Sandimmune)
Berberine can increase serum levels of cyclosporine.
Berberine can reduce metabolism and increase serum levels of cyclosporine. Berberine might inhibit cytochrome P450 3A4 (CYP3A4), which metabolizes cyclosporine.
Anticoagulant/Antiplatelet Drugs
Theoretically, berberine might increase the risk of bleeding when used with anticoagulant or antiplatelet drugs.
In vitro and in vivo research suggest that berberine can inhibit platelet aggregation. Theoretically, berberine might have additive effects when used with anticoagulant and antiplatelet drugs and increase the risk of bleeding.
Antidiabetes Drugs
Theoretically, berberine may increase the risk of hypoglycemia when taken with antidiabetes drugs.
Clinical research shows that berberine may lower blood glucose levels. Theoretically, berberine might have additive effects with antidiabetes drugs and increase the risk of hypoglycemia.
Antihypertensive Drugs
Theoretically, berberine might have additive effects with antihypertensive drugs.
Animal research suggests that berberine can have hypotensive effects. Also, a clinical study suggests that taking berberine in combination with amlodipine can lower systolic and diastolic blood pressure when compared with amlodipine alone.
Cns Depressants
Theoretically, berberine might increase the sedative effects of CNS depressants.
Animal research suggests that berberine may have sedative effects. Theoretically, use of berberine along with CNS depressants might produce additive therapeutic and adverse effects.
Cytochrome P450 2C9 (Cyp2C9) Substrates
Theoretically, berberine might increase serum levels of drugs metabolized by CYP2C9.
Preliminary clinical research shows that berberine can inhibit CYP2C9. Theoretically, taking berberine with drugs metabolized by CYP2C9 might increase drug levels and increase the risk of adverse effects.
Cytochrome P450 2D6 (Cyp2D6) Substrates
Theoretically, berberine might increase serum levels of drugs metabolized by CYP2D6.
In vitro research and preliminary clinical evidence show that berberine can inhibit CYP2D6. Theoretically, use of berberine with drugs metabolized by CYP2D6 might increase drug levels and increase the risk of adverse effects.
Cytochrome P450 3A4 (Cyp3A4) Substrates
Theoretically, berberine might increase serum levels of drugs metabolized by CYP3A4.
In vitro research and preliminary clinical research show that berberine moderately inhibits CYP3A4. Theoretically, use of berberine with drugs metabolized by CYP3A4 might increase drug levels and increase the risk of adverse effects.
Dextromethorphan (Robitussin Dm, Others)
Theoretically, berberine may increase serum levels of dextromethorphan.
Preliminary clinical research shows that berberine can inhibit cytochrome P450 2D6 (CYP2D6) activity and reduce the metabolism of dextromethorphan. This may increase the effects and side effects of dextromethorphan.
Losartan (Cozaar)
Berberine might reduce the therapeutic effects of losartan by decreasing its conversion to its active form.
Preliminary clinical research suggests that berberine can inhibit cytochrome P450 2C9 (CYP2C9) activity and reduce metabolism of losartan.
Metformin (Glucophage)
Theoretically, berberine might increase the therapeutic and adverse effects of metformin.
In vitro and animal studies show that berberine can increase the systemic exposure and half-life of metformin, potentially increasing metformin's effects and side effects. This interaction seems to be most apparent when berberine is administered 2 hours prior to metformin. Taking berberine and metformin at the same time does not appear to increase systemic exposure to metformin.
Midazolam (Versed)
Berberine can reduce metabolism of midazolam, which might increase the risk of severe adverse effects.
Preliminary clinical research shows that berberine can inhibit cytochrome P450 3A4 (CYP3A4) activity and reduce metabolism of midazolam.
Pentobarbital (Nembutal)
Berberine might increase the sedative effect of pentobarbital.
Evidence from animal research shows that berberine can prolong pentobarbital-induced sleeping time. Theoretically, combining berberine and pentobarbital might increase the sedative effects of pentobarbital.
Tacrolimus (Prograf)
Berberine has been associated with increased blood levels of tacrolimus.
In a 16-year-old patient with idiopathic nephrotic syndrome who was being treated with tacrolimus 6.5 mg twice daily, intake of berberine 200 mg three times daily increased the blood concentration of tacrolimus from 8 to 22 ng/mL. Following a reduction of the tacrolimus dose to 3 mg daily, blood levels of tacrolimus decreased to 12 ng/mL.
Acetazolamide
Laboratory studies and initial clinical findings suggest that berberine has the potential to increase acetazolamide concentrations in the body. More research is needed to confirm this interaction.
Uva ursi
Cytochrome P450 2C19 (Cyp2C19) Substrates
Theoretically, uva ursi may decrease the metabolism of CYP2C19 substrates.
In vitro, uva ursi appears to inhibit cytochrome CYP2C19. This effect has not been reported in humans.
Cytochrome P450 3A4 (Cyp3A4) Substrates
Theoretically, uva ursi may decrease the metabolism of CYP3A4 substrates.
In vitro, uva ursi appears to inhibit CYP3A4. This effect has not been reported in humans.
Glucuronidated Drugs
Theoretically, uva ursi may increase levels of drugs metabolized by glucuronidation.
In vitro, uva ursi extract appears to strongly inhibit UDP-glucuronosyltransferase (UGT) 1A1 (UGT1A1). However, uva ursi extract does not appear to inhibit UGT1A1 in animal models. This effect has not been reported in humans.
Lithium
Theoretically, uva ursi may increase lithium levels, necessitating a decrease in dose.
Uva ursi may have diuretic properties. Diuretics may increase lithium reabsorption with sodium in the proximal tubule of the kidney. Theoretically, uva ursi might reduce excretion and increase levels of lithium.
Urinary Acidifying Agents
Effects of uva ursi in the urinary tract may be reduced by urinary acidifying agents.
Uva ursi seems to work best in alkaline urine. Theoretically, taking uva ursi with medications known to acidify the urine may decrease any effects of uva ursi on the urinary tract.
P-Glycoprotein Substrates
Theoretically, uva ursi may alter the levels of drugs transported by P-glycoprotein.
In vitro, uva ursi appears to inhibit the multi-drug transporter protein, P-glycoprotein. This effect has not been reported in humans.
Cranberry
Atorvastatin (Lipitor)
Theoretically, cranberry might increase levels and adverse effects of atorvastatin.
In one case report, a patient taking atorvastatin experienced upper back pain, rhabdomyolysis, and abnormal liver function after drinking cranberry juice 16 ounces daily for 2 weeks. Theoretically, this may have been caused by inhibition of cytochrome P450 3A4 (CYP3A4) enzymes by cranberry juice, as atorvastatin is a CYP3A4 substrate. Creatinine kinase and liver enzymes normalized within 2 weeks of stopping cranberry juice. Patients taking atorvastatin should avoid large quantities of cranberry juice.
Cytochrome P450 3A4 (Cyp3A4) Substrates
Theoretically, cranberry might increase the levels and adverse effects of CYP3A4 substrates.
A case of upper back pain, rhabdomyolysis, and abnormal liver function has been reported for a patient taking atorvastatin, a CYP3A4 substrate, in combination with cranberry juice 16 ounces daily for 2 weeks. Creatinine kinase and liver enzymes normalized within 2 weeks of stopping cranberry juice. Also, animal research suggests that cranberry juice, administered intraduodenally 30 minutes prior to nifedipine, a CYP3A4 substrate, inhibits nifedipine metabolism and increases the area under the concentration-time curve by 1.6-fold compared to control.
Nifedipine (Procardia)
Theoretically, cranberry might increase the levels and adverse effects of nifedipine.
Animal research suggests that cranberry juice, administered intraduodenally 30 minutes prior to nifedipine treatment, inhibits nifedipine metabolism and increases the area under the concentration-time curve by 1.6-fold compared to control. This interaction has not been reported in humans.
Warfarin (Coumadin)
Theoretically, cranberry might increase the levels and adverse effects of warfarin. However, research is conflicting.
There is contradictory evidence about the effect of cranberry juice on warfarin. Case reports have linked cranberry juice consumption to increases in the international normalized ratio (INR) in patients taking warfarin, resulting in severe spontaneous bleeding and excessive postoperative bleeding. Daily consumption of cranberry sauce for one week has also been linked to an increase in INR in one case report. In a small study in healthy young males, taking a high dose of 3 grams of cranberry juice concentrate capsules, equivalent to 57 grams of fruit daily, for 2 weeks produced a 30% increase in the area under the INR-time curve after a single 25-mg dose of warfarin. However, 3 very small clinical studies in patients stabilized on warfarin reported that cranberry juice 250 mL once or twice daily for 7 days (27% cranberry juice or pure cranberry juice) or 240 mL once daily for 14 days does not significantly increase INR or affect plasma warfarin levels. The reasons for these discrepant findings are unclear. It is possible that the form and dose of cranberry may play a role, as cranberry extracts and juices contain different constituents. Additionally, an in vitro study evaluating 5 different cranberry juices found varying effects, with only a cranberry concentrate, and not diluted cranberry juices, inhibiting CYP2C9. However, this concentrate did not inhibit CYP2C9 activity in humans.
Cytochrome P450 2C9 (Cyp2C9) Substrates
Theoretically, cranberry might increase the levels and adverse effects of CYP2C9 substrates. However, research is conflicting.
There is contradictory evidence about the effect of cranberry on CYP2C9 enzymes. In vitro evidence suggests that flavonoids in cranberry inhibit CYP2C9 enzymes. However, clinical research shows that cranberry juice does not significantly affect the levels, metabolism, or elimination of the CYP2C9 substrates flurbiprofen or diclofenac. Also, in patients stabilized on warfarin, drinking cranberry juice 250 mL daily for 7 days does not significantly increase the anticoagulant activity of warfarin, a CYP2C9 substrate. Additional pharmacokinetic research shows that cranberry juice does not increase peak plasma concentrations or area under the concentration-time curve of warfarin.
Diclofenac (Voltaren, Others)
Theoretically, cranberry might modestly increase the levels and adverse effects of diclofenac.
In vitro evidence suggests that cranberry juice inhibits diclofenac metabolism by human liver microsomes. However, drinking cranberry juice does not seem to affect diclofenac metabolism in humans.
Probiotic
Antibiotic Drugs
Theoretically, taking Lactobacillus acidophilus with antibiotic drugs might decrease the effectiveness of L. acidophilus.
L. acidophilus preparations usually contain live and active organisms. Therefore, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and L. acidophilus preparations by at least two hours.
