Why this applies to Fedratinib Hydrochloride

Fedratinib Hydrochloride falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Aegeline interacts with.

Aegeline inhibits cytochrome P450 3A4 (CYP3A4) activity in vitro. So far, this interaction has not been reported in humans. Theoretically, aegeline may increase levels of drugs metabolized by CYP3A4. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), ketoconazole (Nizoral), itraconazole (Sporanox), fexofenadine (Allegra), triazolam (Halcion), and numerous others. Use aegeline cautiously or avoid in patients taking these drugs.

Here's the basic idea with fedratinib (Inrebic). Your liver uses an enzyme called CYP3A4 to clear this medicine out of your body. In lab testing, aegeline appears to slow that same enzyme down. If that happened in a real person, fedratinib could build up to higher levels than expected, which may mean more side effects like nausea, diarrhea, low blood counts (anemia), or fatigue. Fedratinib also carries a serious warning about a brain condition called encephalopathy, so higher levels are not something to take lightly.

That said, this is only a theoretical, test-tube concern and hasn't been shown in people. Honestly, aegeline has been tied to severe liver injury and regulators warned against it, so I'd steer clear regardless. Please check with your oncology team before adding anything.

Aegeline is a compound originally identified in the bael tree (Aegle marmelos) that gained attention because it was added to some weight-loss and workout supplements. It is best known for being linked to serious liver injury in a product called OxyELITE Pro, and U.S. regulators warned against products containing it. There is no good evidence it safely helps with weight loss, and it should generally be avoided.

What Is Fedratinib Used For?

Brand name: Inrebic

Inrebic (fedratinib) is approved to treat adult patients with intermediate-2 or high-risk myelofibrosis (MF) — a type of bone marrow cancer that disrupts normal blood cell production. This includes primary myelofibrosis as well as secondary myelofibrosis that developed after polycythemia vera or essential thrombocythemia.

It is used both in patients who have not been treated before and in those who have previously been treated with ruxolitinib, another myelofibrosis medication.
