BiDil pairs two blood pressure medicines, isosorbide dinitrate (a nitrate that relaxes and widens blood vessels) and hydralazine (which also opens up arteries). Both already work to bring blood pressure down. Agmatine, mostly based on animal studies, may modestly lower blood pressure and heart rate on its own. So the worry here is simply additive: two things pushing in the same direction.

If both lowered your pressure too much, you might feel dizzy, lightheaded, faint, or headachy, especially when standing up. Keep in mind this concern is theoretical and not proven in people. Still, since you are on BiDil for your heart, it is worth running agmatine past your pharmacist or doctor before you start it.
