Alogliptin, Metformin falls under ANTIDIABETES DRUGS — the group Alfalfa interacts with.

Theoretically, alfalfa might increase the risk of hypoglycemia when taken with antidiabetes drugs.
Animal research suggests that alfalfa decreases blood sugar in diabetic mice. Also, in one case report, a diabetic patient experienced hypoglycemia after consuming alfalfa extract. Monitor blood glucose levels closely. Dose adjustments might be necessary.

Here's the gist: Kazano combines two diabetes medicines (alogliptin and metformin) that already work to bring your blood sugar down. Alfalfa, on its own, may have a mild blood-sugar-lowering effect too. Stack the two together and there's a chance your numbers could dip lower than you want, what we call hypoglycemia. That can show up as shakiness, sweating, dizziness, a racing heart, or feeling confused or unusually hungry.

I want to be honest about the evidence here: it's pretty thin. This is mostly based on diabetic mice and a single patient report, not solid human studies, so it's more of a theoretical heads-up than a sure thing. Still, since you're managing diabetes, it's worth checking your glucose more closely if you add alfalfa, and chatting with your pharmacist or doctor before starting it.
