Hydralazine, Hydrochlorothiazide falls under PHOTOSENSITIZING DRUGS — the group Alfalfa interacts with.

Theoretically, concomitant use of alfalfa with photosensitizing drugs might have additive effects.

Here's the gist: your combo pill pairs hydralazine with hydrochlorothiazide, and that water pill (the HCTZ part) is already known to make some people's skin extra sensitive to sunlight. Alfalfa, especially in concentrated supplement doses, may add a little to that effect, possibly because of its chlorophyll content. So in theory, taking the two together could make you sunburn a bit more easily or break out in a rash on sun-exposed skin.

I want to keep this in perspective. This concern comes mostly from animal studies, and it's unclear whether it really matters in people. Food amounts of alfalfa (like sprouts on a sandwich) are unlikely to be an issue. If you do take alfalfa supplements, just be smart with sunscreen and a hat, and run it by your pharmacist or doctor first.
