Here's the gist: your combo pill (amiloride plus hydrochlorothiazide) lowers blood pressure by helping your kidneys clear out extra salt and water. There is some lower-quality clinical evidence that drinking apple juice (and cherry juice), especially in older folks, may nudge blood pressure back up a bit. That could work against what your medicine is trying to do, making your readings less controlled.

Now, an apple you eat as food is very different from drinking glasses of juice, and the whole fruit is a healthy choice. This isn't a dangerous reaction, just something to keep an eye on. If you regularly drink apple juice, mention it to your pharmacist or doctor and keep tabs on your blood pressure rather than changing anything on your own.

About Amiloride and Hydrochlorothiazide

What Is Amiloride and Hydrochlorothiazide Used For?

Brand names: Moduretic, Amilzide, Moduret 25, Amil-Co

Amiloride hydrochloride and hydrochlorothiazide tablets are used for patients with high blood pressure (hypertension) or congestive heart failure who tend to develop low potassium (hypokalemia) when taking a thiazide or other diuretic alone — or for whom keeping potassium levels in a normal range is especially important, such as people taking digoxin or those with significant heart rhythm problems.

The tablets may be used on their own or alongside other blood pressure medicines, such as methyldopa or beta blockers. This combination product is not intended as the first-choice treatment for swelling (edema) or high blood pressure unless low potassium is a specific concern.
