Following the Atkins diet while taking antidiabetes drugs might increase the risk of hypoglycemia. In patients with diabetes, low-carbohydrate diets such as the Atkins diet can reduce the need for antidiabetes drugs; monitor blood glucose closely.

Here's the simple version: when you eat very few carbs on Atkins, your blood sugar naturally drops. Layer that on top of ertugliflozin (Steglatro), which already lowers blood sugar by flushing glucose out through your urine, and the two can add up. The result could be blood sugar that runs lower than you intend, leaving you shaky, sweaty, lightheaded, or unusually tired.

This is mostly a theoretical concern based on expert opinion, and true lows are less common with SGLT2 drugs like this one when taken alone. But there's a separate point worth knowing: very low-carb diets can also nudge ketones up, and SGLT2 inhibitors carry their own small risk of ketoacidosis. Don't change anything on your own. Just check your sugars more often at first and let your doctor or pharmacist know you're starting Atkins so they can guide you.
