Amitriptyline, Perphenazine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Atractylodes interacts with.

Theoretically, atractylodes might decrease the levels of CYP1A2 substrates.
In animals, atractylodes administered at high doses has been shown to induce CYP1A2 activity. This effect has not been shown in humans.

Here's the gist for your Triavil (the amitriptyline plus perphenazine combo). One of the liver enzymes that helps break down these medicines is called CYP1A2. In animal studies given high doses, atractylodes seemed to speed that enzyme up. If that happened in people, it could clear the drug a little faster and make it work less well, meaning your mood symptoms or anxiety might not be as well controlled.

Honestly, though, this is theoretical. It has only been seen in lab and animal research, never confirmed in humans, and it's rated a minor, possible interaction. I wouldn't lose sleep over it. Still, before adding atractylodes, run it by your pharmacist or doctor so they can keep an eye on how you're doing.

Amitriptyline, Perphenazine also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, atractylodes might increase the levels of CYP3A4 substrates.
In animals, atractylodes administered at high doses has been shown to inhibit CYP3A1 activity, which is a homolog to the human CYP3A4 enzyme. This effect has not been shown in humans.

Triavil combines two active medicines: amitriptyline (an antidepressant) and perphenazine (an antipsychotic). Your body partly relies on a liver enzyme called CYP3A4 to break both of them down. In lab and animal studies, atractylodes seemed to slow a similar enzyme, which in theory could let these drugs build up a bit higher than usual.

If that happened, you might notice more of their typical effects, like drowsiness, dry mouth, constipation, dizziness when standing, or a faster heartbeat. Here's the honest part though: this has only been seen at high doses in animals, never proven in people, so I'd call it a low-level, mostly theoretical concern. Still, because these are sedating medicines, it's worth a quick chat with your pharmacist or doctor before adding atractylodes.
