Atractylodes and Aspirin, Butalbital, Caffeine

These two interact in 3 distinct ways. Each one is explained separately below.

Why Are There 3 Interactions?

Atractylodes interacts with whole categories of medication. Aspirin, Butalbital, Caffeine falls into 3 of those categories, so it can interact with Atractylodes in 3 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

Moderate

ANTICOAGULANT/ANTIPLATELET DRUGS

1 of 3

Why this applies to Aspirin, Butalbital, Caffeine

Aspirin, Butalbital, Caffeine falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Atractylodes interacts with.

Theoretically, atractylodes might increase the risk of bleeding when used concomitantly with anticoagulant and antiplatelet drugs.
Laboratory research suggests that atractylenolides II and III, constituents of atractylodes, reduce platelet activation. So far, this has not been shown in humans.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Here's the thing to know about Fiorinal: it contains aspirin, which already thins the blood a bit by making your platelets less sticky. Atractylodes may do something similar. Lab studies on certain compounds in this root (called atractylenolides) hint that they can also calm platelet activity. Stack that on top of aspirin and, in theory, you could bruise more easily or bleed a little longer from a cut, with stomach bleeding being the bigger concern aspirin can cause.

I want to be honest, though: this is only a theoretical worry from test tube and animal work, not anything proven in people. It's worth a quick chat with your pharmacist or doctor before combining them, especially if you ever notice unusual bruising, dark stools, or stomach upset.

CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES

2 of 3

Aspirin, Butalbital, Caffeine also falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES.

Theoretically, atractylodes might decrease the levels of CYP1A2 substrates.
In animals, atractylodes administered at high doses has been shown to induce CYP1A2 activity. This effect has not been shown in humans.

Here's the gist with Fiorinal (aspirin, butalbital, and caffeine). The caffeine piece is partly broken down by a liver enzyme called CYP1A2. In high-dose animal studies, atractylodes seemed to speed up that enzyme, which in theory could clear caffeine a little faster and slightly blunt the small lift caffeine adds to your headache relief. None of the three ingredients here is a prodrug, so we're not talking about losing the aspirin or butalbital effect.

Honestly, this is a minor, theoretical concern that has never been shown in people, so I wouldn't lose sleep over it. If you take this combo regularly, just give your pharmacist or doctor a quick heads-up before adding atractylodes.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

3 of 3

Aspirin, Butalbital, Caffeine also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, atractylodes might increase the levels of CYP3A4 substrates.
In animals, atractylodes administered at high doses has been shown to inhibit CYP3A1 activity, which is a homolog to the human CYP3A4 enzyme. This effect has not been shown in humans.
Severity Minor Likelihood Possible Evidence D

Here's the gist with Fiorinal (aspirin, butalbital, and caffeine). The concern is that atractylodes might slow down a liver enzyme called CYP3A4, which helps clear part of this combination, especially the butalbital. If that enzyme runs slower, the medicine could hang around a little longer and feel stronger. With butalbital that mostly means more drowsiness, grogginess, dizziness, or feeling foggy.

But I want to be honest about how thin this evidence is. The only signal comes from animal cells given very high doses, and it has never been shown in actual people. So in practice, this is more theoretical than real. Still, since butalbital is already sedating, it's worth a quick chat with your pharmacist or doctor before combining them.

About Atractylodes

What is Atractylodes?

Atractylodes
  • Digestive upset and poor appetite
  • Bloating and fluid retention
  • General fatigue or 'weakness'
  • Diarrhea
  • Support within traditional formulas

Atractylodes is a root used for centuries in traditional Chinese, Japanese, and Thai medicine, mostly for digestive complaints and fatigue, often as part of multi-herb formulas. Modern research is still early and mostly from lab and animal studies, so its true benefits in people are not well proven. If you want to try it, talk with a pharmacist or doctor first, and avoid it during pregnancy and breastfeeding.

About Aspirin, Butalbital, and Caffeine

What Is Aspirin, Butalbital, and Caffeine Used For?

Brand name: Fiorinal

This combination is used to relieve the symptoms of tension (or muscle contraction) headache. The supported product is the oral capsule.

There is no evidence that it works or is safe for multiple recurrent headaches. Caution is needed because butalbital is habit-forming and can be abused.
