Aspirin, Pentazocine falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Atractylodes interacts with.

Theoretically, atractylodes might increase the risk of bleeding when used concomitantly with anticoagulant and antiplatelet drugs.
Laboratory research suggests that atractylenolides II and III, constituents of atractylodes, reduce platelet activation. So far, this has not been shown in humans.

Talwin Compound pairs aspirin (a blood thinner that keeps platelets from clumping) with pentazocine (a pain reliever). The concern here is really about the aspirin part. Lab and animal research hints that certain atractylodes compounds may also make platelets a little less sticky. If that small effect carries over into people, stacking it on top of aspirin could, in theory, add up and nudge your bleeding risk higher.

To be honest, this is only a theoretical worry so far. It has not been shown in humans, so I would not panic. Still, keep an eye out for easy bruising, nosebleeds, bleeding gums, or stomach upset. Before combining them, run it by your pharmacist or doctor, especially if you take this regularly.

CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES

Aspirin, Pentazocine also falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES.

Theoretically, atractylodes might decrease the levels of CYP1A2 substrates.
In animals, atractylodes administered at high doses has been shown to induce CYP1A2 activity. This effect has not been shown in humans.

Here's the gist for Talwin Compound, which pairs aspirin with the pain reliever pentazocine. In animal studies, high doses of atractylodes seemed to speed up a liver enzyme called CYP1A2. If that happened in people, it could in theory clear pentazocine a bit faster, which might make it work a little less well for pain. Neither of these is a prodrug, so we're talking about slightly weaker effect, not stronger.

The honest truth is that this is only a theoretical, lab-and-animal finding that has never been shown in humans, and it's rated minor. I wouldn't lose sleep over it. Still, since combo painkillers can be tricky, it's worth a quick mention to your pharmacist or doctor before adding atractylodes.
