Why this applies to Atazanavir, Cobicistat

Atazanavir, Cobicistat falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Atractylodes interacts with.

Theoretically, atractylodes might increase the levels of CYP3A4 substrates.
In animals, atractylodes administered at high doses has been shown to inhibit CYP3A1 activity, which is a homolog to the human CYP3A4 enzyme. This effect has not been shown in humans.

Here's the gist with Evotaz (atazanavir plus cobicistat) and atractylodes. Both of those medicines are cleared by a liver enzyme called CYP3A4. In high-dose animal studies, atractylodes seemed to slow down a similar enzyme. If that happened in people, it could let atazanavir build up a bit, which in theory might mean more side effects like nausea, stomach upset, or yellowing of the eyes and skin (a known atazanavir effect).

The important part: this has only been seen in lab and animal research, never proven in humans, and it's rated a minor, possible interaction. So I wouldn't lose sleep over it. Still, because keeping HIV treatment steady really matters, please run any atractylodes supplement past your pharmacist or doctor before starting it.

What Is Atazanavir and Cobicistat Used For?

Brand name: Evotaz

Evotaz (atazanavir/cobicistat) is used to treat HIV-1 infection. It must always be taken together with other antiretroviral medicines — it is not a complete regimen on its own. It is approved for adults and for pediatric patients who weigh at least 35 kg.

In patients who have been on HIV treatment before (treatment-experienced), how well Evotaz works depends on how much resistance to protease inhibitors the virus has already developed. Your prescriber will review this before starting you on Evotaz.
