Belladonna, Caffeine, Ergotamine, Pentobarbital falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Atractylodes interacts with.

Theoretically, atractylodes might decrease the levels of CYP1A2 substrates.
In animals, atractylodes administered at high doses has been shown to induce CYP1A2 activity. This effect has not been shown in humans.

This combination pill contains caffeine, and caffeine is cleared from your body by a liver enzyme called CYP1A2. In animal studies, large doses of atractylodes seemed to speed up that enzyme. If that happened in people, your body might clear the caffeine a little faster, so you could notice slightly less of its effect. Honestly, that is a small piece of this pill anyway, and the belladonna, ergotamine, and pentobarbital parts are not really driven by this enzyme.

Here is the reassuring part: this is only a theoretical concern from lab and animal work, and it has never been shown in humans. I would not expect a real problem at normal doses. Still, since this medicine treats headaches and has potent ingredients, check with your pharmacist or doctor before adding atractylodes.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Belladonna, Caffeine, Ergotamine, Pentobarbital also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, atractylodes might increase the levels of CYP3A4 substrates.
In animals, atractylodes administered at high doses has been shown to inhibit CYP3A1 activity, which is a homolog to the human CYP3A4 enzyme. This effect has not been shown in humans.

Your medication Micomp PB is a combination product, and one of its key pieces, the ergotamine, is cleared by a liver enzyme called CYP3A4. In lab and animal studies (not in people), high doses of atractylodes appeared to slow that enzyme down a bit. If that were to happen in humans, ergotamine could build up more than usual, which in theory might mean stronger ergot effects like nausea, tingling or cold hands and feet, or worse headaches.

Honestly, this is a minor, theoretical concern based only on animal data, so I wouldn't lose sleep over it. Still, ergotamine has a fairly narrow safe range, so it's worth a quick chat with your pharmacist or doctor before adding atractylodes.

Atractylodes is a root used for centuries in traditional Chinese, Japanese, and Thai medicine, mostly for digestive complaints and fatigue, often as part of multi-herb formulas. Modern research is still early and mostly from lab and animal studies, so its true benefits in people are not well proven.
