Diltiazem, Enalapril falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Atractylodes interacts with.

Theoretically, atractylodes might increase the levels of CYP3A4 substrates.
In animals, atractylodes administered at high doses has been shown to inhibit CYP3A1 activity, which is a homolog to the human CYP3A4 enzyme. This effect has not been shown in humans.

This combo pill pairs diltiazem with enalapril. The diltiazem half is handled by a liver enzyme called CYP3A4. The concern here is that atractylodes might slow that enzyme down, which could let diltiazem build up a bit. If that happened, you might notice more of its usual effects, like a slower heart rate, lower blood pressure, dizziness, or swollen ankles.

Here's the reassuring part: this is only a theoretical worry seen in animal studies given high doses, and it has never been shown in people. So it's a low-level concern, not a red flag. Still, since blood pressure and heart rate matter, check with your pharmacist or doctor before adding atractylodes, and let them know if you feel unusually lightheaded.
