Why this applies to Imipramine

Imipramine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Atractylodes interacts with.

Theoretically, atractylodes might decrease the levels of CYP1A2 substrates.
In animals, atractylodes administered at high doses has been shown to induce CYP1A2 activity. This effect has not been shown in humans.

Here's the gist for imipramine (Tofranil). Your liver uses an enzyme called CYP1A2, among others, to help clear this medicine. In animal studies using high doses, atractylodes seemed to speed that enzyme up. If the same thing happened in people, your body might break down imipramine a little faster, which could lower the amount in your blood and make it work less well for depression or bedwetting.

The important catch: this has only been seen in lab and animal research, never confirmed in humans, and it's rated a minor interaction. So I wouldn't lose sleep over it. Still, since imipramine needs steady levels to work, please run any atractylodes product past your pharmacist or doctor first.

Imipramine also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, atractylodes might increase the levels of CYP3A4 substrates.
In animals, atractylodes administered at high doses has been shown to inhibit CYP3A1 activity, which is a homolog to the human CYP3A4 enzyme. This effect has not been shown in humans.

Here's the gist for imipramine (Tofranil): your liver uses an enzyme called CYP3A4 to help break it down. In animal studies (not people), high doses of atractylodes slowed a similar enzyme. The worry on paper is that if atractylodes blocked this pathway, imipramine could clear out a little slower and build up, which might mean stronger side effects like drowsiness, dry mouth, constipation, dizziness on standing, or a faster heartbeat.

That said, this is rated minor and rests only on lab and animal data, so it has never been proven in humans. I wouldn't lose sleep over it, but since imipramine already carries those side effects, it's smart to mention any new supplement to your pharmacist or doctor first.

Atractylodes is a root used for centuries in traditional Chinese, Japanese, and Thai medicine, mostly for digestive complaints and fatigue, often as part of multi-herb formulas. Modern research is still early and mostly from lab and animal studies, so its true benefits in people are not well proven. If you want to try it, talk with a pharmacist or doctor first, and avoid it during pregnancy and breastfeeding.

About Imipramine

What Is Imipramine Used For?

Brand names: Tofranil, Tofranil PM

Imipramine is used to relieve symptoms of depression. Both Imipramine Hydrochloride tablets and Imipramine Pamoate capsules are approved for this use. Endogenous depression — the kind that seems to arise from within rather than a clear outside cause — tends to respond especially well. It may take one to three weeks before you notice the full benefit.

Imipramine Hydrochloride tablets are also approved as a short-term, add-on treatment to help reduce nighttime bedwetting (nocturnal enuresis) in children aged 6 and older, after a doctor has ruled out physical causes.
