Ranolazine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Bacopa interacts with.

Theoretically, bacopa might increase the levels and adverse effects of CYP3A4 substrates.
Research on the effects of bacopa extracts on CYP3A4 enzymes is conflicting. Some in vitro evidence suggests that bacopa extract can moderately and competitively inhibit CYP3A4, while other in vitro evidence suggests that any effect is unlikely to be clinically significant.

Here's the gist for ranolazine (Ranexa): your liver uses an enzyme called CYP3A4 to clear this medication out. In lab dishes, bacopa has shown some ability to slow that same enzyme down. If that were to happen in real life, ranolazine could build up a bit more than usual, and higher levels can mean more of its typical side effects, things like dizziness, headache, nausea, or constipation.

Now, I want to be honest about how shaky this is. The evidence comes only from test-tube studies, and even those disagree with each other, so this may not matter much in actual people. Still, since ranolazine levels are sensitive to CYP3A4, it's worth a quick chat with your pharmacist or doctor before you pair the two.

Bacopa is an Ayurvedic herb most often used for memory and thinking. Some small studies suggest it may modestly help memory when taken regularly for several weeks, but the evidence is limited and quality varies. What Is Ranolazine Used For?

Brand names: Ranexa, Aspruzyo

Ranolazine treats chronic angina — the recurring chest pain or pressure that happens when the heart isn't getting enough blood and oxygen. It is used when other angina medicines aren't enough on their own.

Both the extended-release tablet (sold as Ranexa and generic ranolazine) and the oral powder form (sold as Aspruzyo Sprinkle) are approved for this use. Ranolazine can be combined with other heart medicines such as beta-blockers, nitrates, calcium channel blockers, anti-platelet drugs, statins, ACE inhibitors, and angiotensin receptor blockers.
