Entrectinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Berberine interacts with.

Theoretically, berberine might increase serum levels of drugs metabolized by CYP3A4.
In vitro research and preliminary clinical research show that berberine moderately inhibits CYP3A4. Theoretically, use of berberine with drugs metabolized by CYP3A4 might increase drug levels and increase the risk of adverse effects.

Here's the situation with these two. Your body relies on a liver enzyme called CYP3A4 to break down and clear entrectinib. Berberine appears to slow that enzyme down, so the worry is that entrectinib could build up higher in your blood than intended. Since entrectinib is an active cancer drug (not a prodrug that needs switching on), more of it hanging around means a greater chance of side effects, things like dizziness, fatigue, swelling, taste changes, constipation, or heart rhythm effects such as a longer QT interval.

Honestly, this is mostly a theoretical concern from lab and early human data, not proven harm. But with a cancer medicine you don't want to gamble. Please loop in your oncologist or pharmacist before adding berberine.

About Entrectinib

What Is Entrectinib Used For?

Brand name: Rozlytrek

Entrectinib (sold as Rozlytrek) treats two types of cancer. Both depend on a specific gene change found by an FDA-approved test.

  • Adults with ROS1-positive metastatic non-small cell lung cancer (NSCLC).
  • Adults and children older than 1 month with solid tumors that have an NTRK gene fusion. The tumor must be metastatic, or surgery must be likely to cause severe harm. It must also have progressed after treatment or have no satisfactory alternative.

The solid tumor use was approved under accelerated approval, based on tumor response rate and how long responses lasted. Continued approval may depend on confirmatory trials. Safety and effectiveness have not been established in children with ROS1-positive NSCLC.
