Why this applies to Entrectinib

Entrectinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Ceylon Leadwort interacts with.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 3A4 (CYP3A4) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP3A4. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), clarithromycin (Biaxin), indinavir (Crixivan), sildenafil (Viagra), triazolam (Halcion), and numerous others.

Here's the situation with these two. Your body clears entrectinib (Rozlytrek) mainly through a liver enzyme called CYP3A4. Plumbagin, a compound in Ceylon Leadwort, has been shown in lab dish studies to slow that same enzyme down. If that happened in a real person, entrectinib could build up to higher levels than intended, which may mean more side effects like dizziness, fatigue, changes in heart rhythm (a longer QT interval), swelling, or numbness and tingling.

I want to be honest though: this is only theoretical, based on test-tube data, and hasn't been seen in actual patients. Still, since entrectinib is a serious cancer drug, I'd play it safe. Please check with your oncologist or pharmacist before adding this supplement.

Ceylon Leadwort (Plumbago zeylanica) is a plant long used in Ayurvedic and other traditional medicine systems, mainly for digestion, skin problems, and pain. Solid human studies are lacking, and the plant contains plumbagin, a compound that can irritate skin and tissues and may be unsafe in larger amounts. What Is Entrectinib Used For?

Brand name: Rozlytrek

Rozlytrek (entrectinib) is used to treat adult patients with ROS1-positive metastatic non-small cell lung cancer (NSCLC), as confirmed by an FDA-approved test. It is also approved for adult and pediatric patients older than 1 month of age who have solid tumors — any cancer type — that carry an NTRK gene fusion detected by an FDA-approved test, provided the tumor has no known acquired resistance mutation, is metastatic or cannot be surgically removed without severe harm, and has progressed after prior treatment or has no satisfactory alternative therapy.

The NTRK gene fusion approval in solid tumors was granted under accelerated approval, meaning it is based on tumor response rates. Continued approval may depend on results from confirmatory clinical trials.
