Theoretically, evodia might have an additive effect with drugs that prolong the QT interval, potentially increasing the risk of ventricular arrhythmias.
Evodia has demonstrated dose-dependent activity as a proarrhythmic agent in animal and in vitro studies. Evodia infusion in animals extends the action duration potential and induces prolongation of the QT interval and Torsade de pointes.

Here's the gist with Evodia and entrectinib (Rozlytrek). Entrectinib can lengthen what's called the QT interval, a measure of your heart's electrical rhythm. Evodia, in lab and animal studies, has done the same thing. Stacking two things that both stretch the QT interval may add up and, in theory, raise the chance of a dangerous heart rhythm. With entrectinib that could show up as a fast or irregular heartbeat, dizziness, lightheadedness, or fainting.

I want to be honest about the strength here: this is theoretical, based only on test tube and animal data for Evodia, not human studies, so don't panic. Still, since entrectinib already carries a real QT warning, I'd skip the Evodia supplement and check with your oncology pharmacist or doctor first.

Entrectinib also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, evodia might increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that evodia extract inhibits hepatic CYP3A4. This effect has not been reported in humans.

Here's the short version: your body relies on a liver enzyme called CYP3A4 to break down and clear entrectinib (Rozlytrek). Lab studies suggest evodia can slow that enzyme down. If that happened in real people, entrectinib could build up in your blood and hit harder than intended, which might mean more of its usual side effects, things like dizziness, fatigue, swelling, an unsteady walk, constipation, or changes in heart rhythm.

I want to be honest about the strength of this, though. So far it's only been seen in test tubes, not in actual patients, so we don't truly know if it matters here. Since entrectinib is a serious cancer medication, please don't add evodia without first checking with your oncologist or pharmacist.

About Entrectinib

What Is Entrectinib Used For?

Brand name: Rozlytrek

Rozlytrek (entrectinib) is used to treat adult patients with ROS1-positive metastatic non-small cell lung cancer (NSCLC), as confirmed by an FDA-approved test. It is also approved for adult and pediatric patients older than 1 month of age who have solid tumors — any cancer type — that carry an NTRK gene fusion detected by an FDA-approved test, provided the tumor has no known acquired resistance mutation, is metastatic or cannot be surgically removed without severe harm, and has progressed after prior treatment or has no satisfactory alternative therapy.

The NTRK gene fusion approval in solid tumors was granted under accelerated approval, meaning it is based on tumor response rates. Continued approval may depend on results from confirmatory clinical trials.
