Pentamidine falls under QT INTERVAL-PROLONGING DRUGS — the group Evodia interacts with.

Theoretically, evodia might have an additive effect with drugs that prolong the QT interval, potentially increasing the risk of ventricular arrhythmias.
Evodia has demonstrated dose-dependent activity as a proarrhythmic agent in animal and in vitro studies. Evodia infusion in animals extends the action duration potential and induces prolongation of the QT interval and Torsade de pointes.

Here's the concern with pairing Evodia and Pentamidine. Pentamidine is already known to affect your heart's electrical rhythm, something we call QT prolongation. In lab and animal studies, Evodia has shown a similar effect, lengthening that same electrical interval. Put the two together and they could theoretically add up, raising the small but real risk of a dangerous irregular heartbeat called Torsade de pointes.

I want to be honest about the evidence here: this is based on test tube and animal research, not human cases, so it's more of a caution than a proven problem. Still, since Pentamidine on its own can disturb heart rhythm, I'd play it safe. Please check with your doctor or pharmacist before combining them.

Pentam 300 (pentamidine isethionate for injection) is indicated for the treatment of pneumonia caused by Pneumocystis carinii (PCP), a potentially life-threatening lung infection that most commonly affects people with weakened immune systems.

Because of its serious risks, Pentam 300 should only be given to patients in whom Pneumocystis carinii infection has actually been confirmed.
