Selpercatinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Beta-methylphenethylamine (bmpea) interacts with.

Theoretically, BMPEA might increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that BMPEA inhibits CYP3A4 by up to 37%.

Here's the gist for Retevmo (selpercatinib): your liver clears it largely through an enzyme called CYP3A4. In a test-tube study, BMPEA blocked that enzyme by about a third. If that holds true in people, selpercatinib could be cleared more slowly and build up in your body, which may mean stronger effects and more side effects, things like high blood pressure, liver enzyme changes, diarrhea, swelling, or heart rhythm (QT) changes.

I want to be straight with you though, this is only a theoretical concern based on lab data, not real patients. BMPEA is also a sketchy, unapproved stimulant that can raise heart rate and blood pressure on its own, which isn't great alongside a cancer therapy. I'd skip it and check with your oncologist or pharmacist first.

What Is Selpercatinib Used For?

Brand name: Retevmo

Selpercatinib treats cancers driven by changes in the RET gene — either a gene fusion (where RET joins abnormally with another gene) or a RET mutation (a change in the gene itself). Your doctor will confirm one of these changes with an FDA-approved test before prescribing it.

It is approved for adults with RET fusion-positive non-small cell lung cancer; adults and children 2 and older with RET-mutant medullary thyroid cancer or RET fusion-positive thyroid cancer; and adults and children 2 and older with other advanced solid tumors that have a RET gene fusion and have not responded to prior treatment.
