CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Acetaminophen, Caffeine, Chlorpheniramine, Hydrocodone, Phenylephrine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Bitter Orange interacts with.
Small clinical studies suggest that single or multiple doses of freshly squeezed bitter orange juice 200-240 mL can inhibit CYP3A4 metabolism of drugs, causing increased drug levels and potentially increasing the risk of adverse effects. However, the extent of the effect of bitter orange on CYP3A4-mediated drug interactions is unknown. Some evidence suggests that bitter orange selectively inhibits intestinal CYP3A4, but not hepatic CYP3A4. Its effect on P-glycoprotein, which strongly overlaps with CYP3A4 interactions, is unclear. One small clinical study shows that drinking 8 ounces of freshly squeezed bitter orange juice has no effect on cyclosporine, which seems to be more dependent on hepatic CYP3A4 and P-glycoprotein than intestinal CYP3A4.

This combo product has a lot going on, but the piece that matters here is the hydrocodone, which your body partly processes through an enzyme called CYP3A4. Bitter orange can mildly block that enzyme in your gut, so in theory a little more hydrocodone could reach your bloodstream. That might mean more of the usual opioid effects, like extra drowsiness, dizziness, constipation, or slowed breathing.
I'd also flag the stimulant angle. This product already contains caffeine and phenylephrine, and bitter orange's synephrine is a stimulant too, so stacking them could nudge your heart rate and blood pressure up. The CYP3A4 evidence is fairly weak, and food amounts of bitter orange likely don't matter, but concentrated supplements are a different story. Here's the thing about Hycomine Compound: it already contains two stimulant-type ingredients, caffeine and phenylephrine (a decongestant that tightens blood vessels). Bitter orange brings its own stimulant, synephrine, which acts a lot like adrenaline. Stack them together and you can pile on the same effects, so the worry here is mostly about your heart and blood pressure, not about the painkiller part.
Realistically, the combo could nudge up your blood pressure and heart rate, or leave you feeling jittery, anxious, or with a racing or pounding heartbeat. This concern is largely theoretical and rated moderate, but if you have high blood pressure or any heart condition, it's worth more caution. Small clinical studies show that taking bitter orange in combination with caffeine can increase blood pressure and heart rate in otherwise healthy normotensive adults. Theoretically, this might increase the risk of serious cardiovascular adverse effects.

Here's the thing to watch with Hycomine Compound: it already contains caffeine, plus phenylephrine, which is a decongestant that also nudges up heart rate and blood pressure. Bitter orange brings its own stimulant (synephrine) to the party. Small studies show that combining bitter orange with caffeine can push blood pressure and heart rate higher in healthy people, so stacking it on top of this combo product means more total stimulant load.
In real life, you might feel jittery, notice a racing or pounding heartbeat, or get a headache. CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES

In vitro research shows that octopamine, a constituent of bitter orange, weakly inhibits CYP2D6 enzymes. This effect has not been reported in humans.

This combo product (Hycomine Compound) packs several active ingredients, and one of them, hydrocodone, is worth a closer look. Hydrocodone is a prodrug, meaning your body uses an enzyme called CYP2D6 to convert it into its stronger active form. Lab studies suggest a compound in bitter orange may weakly slow that same enzyme. In theory, that could mean a bit less pain relief from the hydrocodone, though this has never actually been shown to happen in real people.
Honestly, my bigger concern is the overlapping stimulant load. Bitter orange's synephrine plus the caffeine already in this product can stack up and nudge your heart rate and blood pressure higher. If you take this medicine, just run the bitter orange by your pharmacist or doctor first.
