One case report suggests that taking bitter orange in combination with other stimulants such as caffeine might prolong the QT interval in some patients.

Here's the concern with pairing bitter orange and amiodarone. Amiodarone already affects your heart's electrical timing by lengthening what's called the QT interval. Bitter orange contains synephrine, a stimulant that can nudge your heart rate and blood pressure up, and one case report hints it may also stretch the QT interval, especially when mixed with caffeine. Stacking these two could, in theory, add up and raise the risk of a dangerous heart rhythm.

To be fair, this is based on weak, anecdotal evidence, not strong studies. But amiodarone is a serious heart medicine with a narrow safety margin, so I'd play it cautious. Please check with your pharmacist or doctor before using any bitter orange or weight-loss supplement.

Amiodarone also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Bitter orange might increase levels of drugs metabolized by CYP3A4.
Small clinical studies suggest that single or multiple doses of freshly squeezed bitter orange juice 200-240 mL can inhibit CYP3A4 metabolism of drugs, causing increased drug levels and potentially increasing the risk of adverse effects. However, the extent of the effect of bitter orange on CYP3A4-mediated drug interactions is unknown. Some evidence suggests that bitter orange selectively inhibits intestinal CYP3A4, but not hepatic CYP3A4. Its effect on P-glycoprotein, which strongly overlaps with CYP3A4 interactions, is unclear. One small clinical study shows that drinking 8 ounces of freshly squeezed bitter orange juice has no effect on cyclosporine, which seems to be more dependent on hepatic CYP3A4 and P-glycoprotein than intestinal CYP3A4.

Here's the gist for amiodarone: your liver and gut use an enzyme called CYP3A4 to help clear it from your body. Bitter orange, the citrus stimulant found in many weight-loss and energy supplements, can block some of that enzyme (mostly in the gut). If that happens, more amiodarone may stick around than expected, which could nudge its effects up.

With amiodarone that matters, because it's a tricky drug with a narrow safety window. Higher levels can mean more side effects like a very slow heartbeat, dizziness, or new rhythm problems. The evidence here is modest and the exact size of the effect isn't well known, so I don't want to scare you. Bitter orange is also a stimulant that can raise heart rate and blood pressure on its own, which isn't ideal with a heart-rhythm drug. Please run this by your pharmacist or doctor before combining them.

Amiodarone is prescribed for adults with serious, life-threatening heart-rhythm problems that haven't responded to other medications. Specifically, it's used for two dangerous arrhythmias (abnormal heartbeats): recurrent ventricular fibrillation (a chaotic, potentially fatal heart rhythm) and recurrent ventricular tachycardia (a dangerously fast heart rhythm that causes unstable blood pressure).

Because amiodarone comes with significant risks, doctors reserve it for situations where other options haven't worked or can't be tolerated. It is not a first-choice medication — it's a last-resort tool for serious, life-threatening conditions.
