Bitter Orange and Caffeine , Sodium Benzoate

These two interact in 3 distinct ways. Each one is explained separately below.

Why Are There 3 Interactions?

Bitter Orange interacts with whole categories of medication. Caffeine , Sodium Benzoate falls into 3 of those categories, so it can interact with Bitter Orange in 3 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

Moderate

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

1 of 3

Why this applies to Caffeine , Sodium Benzoate

Caffeine , Sodium Benzoate falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Bitter Orange interacts with.

Bitter orange might increase levels of drugs metabolized by CYP3A4.
Small clinical studies suggest that single or multiple doses of freshly squeezed bitter orange juice 200-240 mL can inhibit CYP3A4 metabolism of drugs, causing increased drug levels and potentially increasing the risk of adverse effects. However, the extent of the effect of bitter orange on CYP3A4-mediated drug interactions is unknown. Some evidence suggests that bitter orange selectively inhibits intestinal CYP3A4, but not hepatic CYP3A4. Its effect on P-glycoprotein, which strongly overlaps with CYP3A4 interactions, is unclear. One small clinical study shows that drinking 8 ounces of freshly squeezed bitter orange juice has no effect on cyclosporine, which seems to be more dependent on hepatic CYP3A4 and P-glycoprotein than intestinal CYP3A4.

Here's the thing to keep in mind with this combo. Bitter orange may mildly block CYP3A4, an enzyme that helps clear certain drugs, so in theory it could let caffeine linger a bit longer in your system. With caffeine and sodium benzoate, that could mean a stronger or longer-lasting caffeine effect: a racing or pounding heart, jitteriness, trouble sleeping, anxiousness, or a bump in blood pressure.

What worries me more is that bitter orange contains synephrine, its own stimulant. So you may be stacking one stimulant on top of another, which is the bigger concern here. The enzyme part is based on small, lower-quality studies, so it isn't certain. Please check with your pharmacist or doctor before combining these, especially if you have heart issues or high blood pressure.

STIMULANT DRUGS

Caffeine , Sodium Benzoate also falls under STIMULANT DRUGS.

Theoretically, bitter orange might increase the risk of hypertension and adverse cardiovascular effects when taken with stimulant drugs.
Bitter orange appears to have stimulant effects.

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the simple version: caffeine and bitter orange are both stimulants, so putting them together is a bit like doubling up. The synephrine in bitter orange acts similar to your body's adrenaline, and caffeine revs things up too. Stacked, they can push your heart rate and blood pressure higher than either one alone.

For most healthy folks the effect is mild, but you might notice a racing or pounding heart, jitteriness, trouble sleeping, or feeling anxious. If you already have high blood pressure or heart issues, this combo deserves real caution. The evidence here is modest, but it's worth a quick chat with your pharmacist or doctor before pairing them, especially at concentrated supplement doses.

CAFFEINE

3 of 3
Bitter orange might increase blood pressure and heart rate when taken with caffeine.
Small clinical studies show that taking bitter orange in combination with caffeine can increase blood pressure and heart rate in otherwise healthy normotensive adults. Theoretically, this might increase the risk of serious cardiovascular adverse effects.
Severity Moderate Likelihood Possible Evidence B

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the thing to keep in mind: caffeine and sodium benzoate is basically a caffeine injection or solution, a pure stimulant. Bitter orange is also a stimulant, thanks to its synephrine. So you're really stacking one stimulant on top of another. A few small clinical studies found that this combination can nudge up your blood pressure and heart rate more than either one alone.

For most healthy folks the effect is mild, but you might notice a faster or pounding heartbeat, jitteriness, anxiety, or trouble sleeping. If you already have high blood pressure or any heart concerns, this pairing matters more. The evidence here is moderate, not rock-solid, but it's worth a quick chat with your pharmacist or doctor before combining the two.

“Caffeine and sodium benzoate is itself a stimulant medication, so the supplements most likely to cause trouble are the ones doing the same job. Seven of the 208 ingredients on file rate major, several of them stimulant herbs such as ephedra that can pile extra blood pressure and heart strain onto a drug already revving the nervous system.”

About Bitter Orange

What is Bitter Orange?

Bitter Orange
  • Weight loss
  • Athletic performance and energy
  • Digestive upset
  • Appetite control
  • Aromatherapy for stress (neroli oil)

Bitter orange is a citrus fruit whose extracts contain synephrine, a mild stimulant often added to weight-loss and energy supplements. Evidence that it works for weight loss or performance is limited, and because it can raise heart rate and blood pressure, people with heart problems, high blood pressure, or who are pregnant should avoid it. Talk to your pharmacist or doctor before using it, especially if you take other stimulants.
