Mitapivat falls under CYTOCHROME P450 3A4 (CYP3A4) INDUCERS — the group Borage interacts with.

Theoretically, taking borage with drugs that induce CYP3A4 might increase levels of pyrrolizidine alkaloid (PA) toxic metabolites.
Although borage seed oil contains little to no PAs, some borage plant parts, such as the leaf, flower, and seed, can contain hepatotoxic PAs. Hepatotoxic PAs are substrates of CYP3A4, which converts these chemicals into toxic metabolites. Tell patients to avoid borage preparations that are not certified and labeled as hepatotoxic PA-free.

Here's the key thing to know about this pairing: it's really about safety for your liver, not about how well your Mitapivat (Pyrukynd) works. Mitapivat can nudge up the activity of a liver enzyme called CYP3A4. That same enzyme is what turns the pyrrolizidine alkaloids (PAs) found in some borage plant parts (leaf, flower, and seed) into harmful, liver-toxic byproducts. So in theory, taking borage alongside Mitapivat could mean more of those toxic byproducts get made.

I want to be honest that this is a theoretical concern based on lab studies, not real patient cases, so don't panic. The practical move is simple: only use borage products clearly labeled 'PA-free,' since properly purified borage seed oil contains little to no PAs. Please run any borage product past your pharmacist or doctor first, especially since you're managing a blood disorder.

What Is Mitapivat Used For?

Brand name: Pyrukynd

Mitapivat treats two distinct types of anemia in adults. Pyrukynd is used to treat hemolytic anemia (anemia caused by red blood cells breaking down too quickly) in adults with pyruvate kinase (PK) deficiency, a rare inherited red blood cell disorder. Aqvesme is used to treat anemia in adults with alpha- or beta-thalassemia, inherited blood conditions that reduce normal hemoglobin production.

These are separate approvals for separate products — Pyrukynd and Aqvesme are not interchangeable, and each is intended for its specific condition.
