Why this applies to Panobinostat

Panobinostat falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Breadfruit interacts with.

Theoretically, breadfruit might inhibit cytochrome P450 3A4 (CYP3A4) and increase levels of drugs metabolized by these enzymes; until more is known, use with caution. In vitro, the breadfruit constituent artocarpin can inhibit CYP3A4 enzymes. Also, animal research shows that breadfruit leaf extract can weakly inhibit CYP3A4 enzymes. However, it is unclear if this inhibitory activity causes a clinically meaningful effect on the metabolism of CYP3A4 substrates.
Some drugs metabolized by CYP3A4 include amitriptyline (Elavil), amiodarone (Cordarone), citalopram (Celexa), felodipine (Plendil), lansoprazole (Prevacid), ondansetron (Zofran), prednisone (Deltasone), sertraline (Zoloft), and many others.

Here's the gist for Farydak (panobinostat): your liver uses an enzyme called CYP3A4 to help break this medicine down. Lab and animal studies hint that a compound in breadfruit (artocarpin) might gently block that same enzyme. If that happened in people, panobinostat could clear a little slower and build up, which in theory could mean more of its known side effects like diarrhea, nausea, fatigue, low blood counts, or heart rhythm changes.

But honestly, the evidence here is thin. This is based on test-tube and animal work, not real patients, and eating breadfruit as food is very different from concentrated leaf teas or extracts. Since panobinostat is a serious cancer drug, please run any breadfruit supplements past your oncologist or pharmacist first.

What Is Panobinostat Used For?

Brand name: Farydak

  • Panobinostat is used in combination with bortezomib (Velcade) and dexamethasone to treat people with multiple myeloma (a type of cancer of the bone marrow) who have already been treated two other medications, including bortezomib (Velcade). Panobinostat is in a class of medications called histone deacetylase (HDAC) inhibitors. It works by killing cancer cells.
