Why this applies to Cabotegravir, Rilpivirine
Cabotegravir, Rilpivirine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Bulbine Natalensis interacts with.
In vitro research shows that Bulbine natalensis stem extract induces CYP3A4 mRNA expression and enzyme activity. Theoretically, this may increase the metabolism of CYP3A4 substrates, but this has not been studied in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the concern with Cabenuva (the long-acting cabotegravir and rilpivirine injection). The rilpivirine part is partly broken down by a liver enzyme called CYP3A4. In lab studies (not in people), Bulbine natalensis appears to speed that enzyme up. If that held true in the body, your medication could be cleared faster than intended, which might lower its levels and weaken how well it controls HIV.
That really matters here, because keeping HIV suppressed depends on steady drug levels, and dropping too low can allow the virus to rebound or develop resistance. I'll be honest though, this is theoretical and based only on test-tube work, so we don't truly know the real-world effect. Given the stakes (plus Bulbine's possible liver concerns), please talk with your HIV provider or pharmacist before adding it.

CABENUVA (cabotegravir + rilpivirine) is used to treat HIV-1 infection in adults and adolescents 12 years and older who weigh at least 35 kg (about 77 lbs). It is not a starter treatment — it's designed for people who are already virologically suppressed, meaning their HIV is currently undetectable on a stable daily pill regimen and who have no history of treatment failure.
The goal is to keep the virus fully suppressed while freeing the person from taking a pill every day. Because it stays active in the body for weeks after each injection, strict adherence to the injection schedule is essential.
