Mephenytoin falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Bulbine Natalensis interacts with.

Theoretically, Bulbine natalensis stem might reduce the levels and clinical effects of CYP2C9 substrates.
In vitro research shows that Bulbine natalensis stem extract induces CYP2C9 mRNA expression and enzyme activity. Theoretically, this may increase the metabolism of CYP2C9 substrates; however, this has not been studied in humans.

Here's the gist for Mephenytoin (Mesantoin), an older seizure medicine. Your liver uses an enzyme called CYP2C9 to help clear this drug. In lab-dish studies, Bulbine natalensis seemed to rev up that enzyme, which could mean your body breaks down mephenytoin faster than usual.

If that happened, your mephenytoin levels could drop and the medicine might not control your seizures as well. That said, this concern comes only from test-tube research, not from people, so we honestly don't know if it plays out in real life. Because seizure control is something you don't want to gamble with, please don't add Bulbine natalensis without first checking with your pharmacist or doctor.
