Phenobarbital, Sodium Nitrate falls under CYTOCHROME P450 3A4 (CYP3A4) INDUCERS — the group Butterbur interacts with.

Theoretically, CYP3A4 inducers might increase the toxicity of butterbur.
The pyrrolizidine alkaloid (PA) constituents found in some butterbur products are substrates of CYP3A4. Theoretically, drugs that induce CYP3A4 might increase the conversion of PAs to toxic metabolites.

Here's the angle to keep in mind with this pairing. Phenobarbital is a strong CYP3A4 inducer, meaning it revs up that liver enzyme system. The worry isn't that butterbur changes your phenobarbital. It's the other direction: certain butterbur products contain pyrrolizidine alkaloids (PAs), and your liver can turn those PAs into harmful byproducts. By speeding up that enzyme, phenobarbital could theoretically ramp up the production of those toxic metabolites, putting more stress on your liver.

I'll be honest, this is a theoretical concern based on how the chemistry works, not real-world reports. The simplest safeguard is choosing a PA-free (PA-free certified) butterbur product, which removes the risky compounds entirely. Please run it by your pharmacist or doctor first, and never adjust your phenobarbital on your own.
