Theoretically, cannabidiol might have additive effects if used with other CNS depressants.
Preliminary clinical research, case reports, and animal studies suggest that high dose cannabidiol has sedative and hypnotic effects. Theoretically, concomitant use of cannabidiol with drugs with sedative and anesthetic properties may cause additive therapeutic and adverse effects.

Here's the gist for using CBD alongside buprenorphine (Belbuca). Buprenorphine is an opioid pain reliever that already calms your central nervous system, and some research suggests that higher doses of CBD can have their own sedating effect. Put the two together and those calming effects may stack up. That could mean feeling extra drowsy, dizzy, foggy-headed, or more tired than usual.

I want to be straight with you, though: this concern is mostly theoretical, based on how these substances behave rather than strong human studies. Still, with an opioid involved, it's smart to play it safe. Don't start or stop anything on your own. Check in with your pharmacist or doctor first, and pay attention to how sleepy or unsteady you feel, especially when you begin CBD.

In vitro and animal research shows that cannabidiol inhibits CYP3A4. In human studies and case reports, cannabidiol has been associated with an increase in plasma levels of the CYP3A4 substrates zonisamide, tacrolimus, everolimus, citalopram, midazolam, and methadone.

Here's the gist for these two together. Your body uses a liver enzyme called CYP3A4 to help clear buprenorphine (Belbuca). CBD can slow that enzyme down, so buprenorphine may not break down as quickly and its levels in your blood can creep up.

When buprenorphine builds up, you may notice more of its usual effects: extra drowsiness, dizziness, nausea, constipation, or slowed, shallow breathing. We have solid human evidence that CBD raises levels of similar drugs, so this isn't just a lab theory. That doesn't mean you can't use both, but please loop in your pharmacist or prescriber first. Don't start, stop, or adjust anything on your own, and let someone know right away if you feel unusually sleepy or hard to wake.
