Why this applies to Cyclophosphamide

Cyclophosphamide falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Cannabidiol (cbd) interacts with.

Cannabidiol may increase levels of drugs metabolized by CYP2C19.
Research shows that cannabidiol inhibits CYP2C19. In clinical studies and case reports, cannabidiol use resulted in significant increases in the serum levels of topiramate, methadone, citalopram, omeprazole, and N-desmethylclobazam, the primary active metabolite of clobazam. These chemicals are metabolized by CYP2C19. Concomitant use of cannabidiol with CYP2C19 substrates may increase the risk for adverse effects from these substrates.

Here's an important wrinkle with cyclophosphamide: it's actually a prodrug. It doesn't do much until your liver enzymes, including CYP2C19, convert it into its active, cancer-fighting form. Because CBD can block CYP2C19, the concern here runs the opposite direction from most drugs. Instead of cyclophosphamide piling up, slowing that enzyme could mean less of the drug gets switched on, which may make your chemo work less effectively.

The science on this exact pairing is still limited, so I won't overstate it. But cyclophosphamide is too important to leave to guesswork. Please don't start, stop, or add any CBD product without first checking with your oncologist or pharmacist, who can weigh this against your specific treatment plan.

In vitro and animal research shows that cannabidiol inhibits CYP3A4. In human studies and case reports, cannabidiol has been associated with an increase in plasma levels of the CYP3A4 substrates zonisamide, tacrolimus, everolimus, citalopram, midazolam, and methadone.

Here's a wrinkle worth knowing: cyclophosphamide is actually a prodrug. It arrives in your body inactive and has to be switched on by liver enzymes, including CYP3A4, before it can fight cancer. CBD tends to slow CYP3A4 down. In theory, that could shift how much active drug your body makes, which is why this pairing is flagged as a real concern with chemotherapy.

Because cyclophosphamide's dosing is so carefully calibrated, even modest changes in how it's processed could matter, potentially affecting how well it works or adding to side effects like nausea, low blood counts, or bladder irritation. Please don't start, stop, or add CBD on your own. Loop in your oncology team or pharmacist first so they can guide you safely.

About Cyclophosphamide

What Is Cyclophosphamide Used For?

Brand names: Cytoxan, Neosar

Cyclophosphamide is used to treat many types of cancer in adults and children, including malignant lymphomas (such as Hodgkin's disease and Burkitt's lymphoma), multiple myeloma, several forms of leukemia, breast cancer, ovarian cancer, neuroblastoma, retinoblastoma, and advanced mycosis fungoides (a type of skin lymphoma). It is often given alongside other chemotherapy drugs rather than on its own.

Oral and injectable powder formulations (but not the Cyclophosphamide Injection solution concentrate) are also approved for minimal change nephrotic syndrome — a kidney condition — in children who have not responded well to, or cannot tolerate, steroid therapy. The injectable solution concentrate is approved for adult patients only due to its alcohol content, and is not approved for use in children.
