Theoretically, CYP3A4 inhibitors might increase cannabidiol levels.
Cannabidiol is a substrate of CYP3A4 enzymes. Theoretically, drugs that inhibit CYP3A4 enzymes might increase levels of cannabidiol, increasing its effects and adverse effects.

Here's the thing to know about Prezcobix (darunavir plus cobicistat) and CBD. The cobicistat part is a strong booster that blocks a liver enzyme called CYP3A4. That same enzyme helps your body break down CBD. So when you take them together, your CBD can be cleared more slowly and quietly build up in your system.

What that may look like in real life is stronger CBD effects than you expect, things like drowsiness, lightheadedness, dry mouth, diarrhea, or changes in appetite. This is a theoretical concern based on how the drugs work, not from solid human studies, so don't panic. Keep your HIV regimen exactly as prescribed, and check with your pharmacist or doctor before adding any CBD product.

Cannabidiol may increase levels of drugs that are metabolized by CYP3A4.
In vitro and animal research shows that cannabidiol inhibits CYP3A4. In human studies and case reports, cannabidiol has been associated with an increase in plasma levels of the CYP3A4 substrates zonisamide, tacrolimus, everolimus, citalopram, midazolam, and methadone.

Here's the situation with Prezcobix (darunavir plus cobicistat) and CBD. Your liver uses an enzyme called CYP3A4 to break down darunavir, and CBD can gently block that enzyme. When the enzyme is slowed, darunavir can build up a bit more than expected. There's a twist worth knowing: cobicistat is in this combo on purpose to boost darunavir levels, so adding CBD pushes in that same upward direction.

Higher darunavir levels could mean more side effects like nausea, diarrhea, headache, or rash. This isn't a reason to panic, and it's certainly not a reason to skip your HIV medicine, which you should keep taking exactly as prescribed. Before adding any CBD product, please run it by your pharmacist or doctor first so they can keep an eye on things.

What Is Darunavir and Cobicistat Used For?

Brand name: Prezcobix

Prezcobix and Prezcobix Ped are used to treat HIV-1 (human immunodeficiency virus type 1) infection. They are always used together with other antiretroviral (anti-HIV) medicines — never alone. Both products are approved for people who are new to HIV treatment (treatment-naïve) and for those who have taken HIV medicines before (treatment-experienced), as long as certain darunavir resistance mutations are not present.

Prezcobix tablets are approved for adults and pediatric patients weighing at least 40 kg. Prezcobix Ped tablets for oral suspension extend coverage to pediatric patients 3 years of age and older weighing at least 15 kg.
