CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES

Dextromethorphan, Guaifenesin falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Cannabidiol (cbd) interacts with.
In vitro research shows that cannabidiol inhibits CYP2D6. Theoretically, concomitant use of cannabidiol with CYP2D6 substrates might increase the risk for adverse effects from these substrates. However, a clinical crossover trial in healthy adults shows that cannabidiol does not inhibit dextromethorphan, a substrate of CYP2D6.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the situation with your cough and chest medicine. The dextromethorphan part is cleared from your body by a liver enzyme called CYP2D6. In a test tube, CBD looked like it could slow that enzyme down, which in theory might let dextromethorphan build up and cause more of its usual effects, like drowsiness, dizziness, or feeling a bit foggy. The guaifenesin piece isn't really involved here.
The reassuring part: an actual study in healthy people tested CBD with dextromethorphan and found CBD did not meaningfully block its breakdown. So this is mostly a theoretical concern, not something seen in real life. Still, if you take a strong CBD product regularly, it's worth a quick word with your pharmacist or doctor.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
2 of 2Dextromethorphan, Guaifenesin also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.
In vitro and animal research shows that cannabidiol inhibits CYP3A4. In human studies and case reports, cannabidiol has been associated with an increase in plasma levels of the CYP3A4 substrates zonisamide, tacrolimus, everolimus, citalopram, midazolam, and methadone.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Here's the simple version: CBD can slow down one of your liver's cleanup enzymes (called CYP3A4). The dextromethorphan part of this cough medicine leans partly on that pathway, so adding CBD could let a little more of it linger in your system than usual. Guaifenesin (the mucus thinner) isn't really affected here, so the focus is on the dextromethorphan.
If levels creep up, you might feel more of dextromethorphan's usual side effects, like drowsiness, dizziness, or a foggy, lightheaded feeling. For occasional, short-term cough use this often isn't a big deal, but the safest move is to run it by your pharmacist or doctor before combining them, especially if you use CBD daily.
