Here's the gist for this pairing: Dronedarone (Multaq) slows down a liver enzyme called CYP3A4, and that same enzyme is one of the main ways your body clears CBD. So when the two are taken together, CBD can break down more slowly and build up to higher levels than expected. That means more of CBD's typical effects, like drowsiness, tiredness, diarrhea, or changes in appetite, could show up.

I'll be honest with you, this is a theoretical concern based on how the drugs work, not something proven in studies, so I don't want you to panic. Still, since dronedarone is a heart rhythm medicine you don't want to mess with, please loop in your pharmacist or doctor before adding any CBD product.

In vitro and animal research shows that cannabidiol inhibits CYP3A4. In human studies and case reports, cannabidiol has been associated with an increase in plasma levels of the CYP3A4 substrates zonisamide, tacrolimus, everolimus, citalopram, midazolam, and methadone.

Here's the simple version: your liver uses an enzyme called CYP3A4 to clear dronedarone out of your body. CBD can slow that same enzyme down. So if you add CBD, dronedarone may not be broken down as quickly and its levels can creep up higher than intended.

With dronedarone, higher levels matter because it's a heart rhythm medicine. More drug in your system could mean a bigger chance of side effects like a slower heart rate, dizziness, stomach upset, or changes on your heart tracing (a longer QT interval). This is a real, well-documented interaction, so please don't start CBD without checking with your pharmacist or cardiologist first. They can decide if it's safe and whether any monitoring is needed.

Theoretically, cannabidiol might increase levels of drugs metabolized by CYP2C8.
In vitro research shows that cannabidiol inhibits CYP2C8. However, this interaction has yet to be reported in humans. Until more is known, use with caution. Theoretically, concomitant use of cannabidiol with CYP2C8 substrates might increase the risk for adverse effects from these substrates.

Here's the gist with these two together. Lab studies suggest that CBD can block an enzyme called CYP2C8, which is one of the pathways your body uses to clear dronedarone. If that enzyme slows down, dronedarone could build up a little more than usual. Since dronedarone is a heart rhythm drug with a fairly tight safety window, even modest increases are worth taking seriously, with effects like a slower heart rate, dizziness, fatigue, or stomach upset.

The honest caveat: this is only a theory based on test-tube data, and it has not been confirmed in real people. So I wouldn't panic, but I also wouldn't combine them on your own. Please run it by your pharmacist or cardiologist first so they can keep an eye on your heart and how you feel.

What Is Dronedarone Used For?

Brand name: Multaq

Multaq (dronedarone) tablets are used to reduce the risk of being hospitalized for atrial fibrillation (AF) — an irregular, often rapid heart rhythm. It is approved for adults who are currently in normal sinus rhythm but have a history of paroxysmal (comes and goes) or persistent atrial fibrillation.

Multaq is not appropriate for everyone with AF. It must not be used in people with permanent atrial fibrillation or in those with recently worsened heart failure requiring hospitalization — these patients face a doubled risk of death, stroke, and hospitalization if they take it.
