Cannabidiol is a substrate of CYP3A4 enzymes. Theoretically, drugs that inhibit CYP3A4 enzymes might increase levels of cannabidiol, increasing its effects and adverse effects.

Here's the thing to understand about this pairing: larotrectinib (Vitrakvi) is a mild blocker of the liver enzyme CYP3A4, and your body uses that same enzyme to clear CBD. So in theory, taking them together could slow down how fast you break down CBD, letting CBD build up a bit. The concern is mostly with the CBD side, not the cancer medicine itself. Higher CBD levels could mean more of CBD's typical effects, like drowsiness, tiredness, diarrhea, or changes in appetite.

I want to be honest that this is a theoretical concern based on how these drugs work on paper, not something proven in studies. Still, since you're on a serious cancer treatment, please loop in your oncology team or pharmacist before adding any CBD product.

In vitro and animal research shows that cannabidiol inhibits CYP3A4. In human studies and case reports, cannabidiol has been associated with an increase in plasma levels of the CYP3A4 substrates zonisamide, tacrolimus, everolimus, citalopram, midazolam, and methadone.

Here's the short version: your body clears larotrectinib using a liver enzyme called CYP3A4, and CBD can slow that enzyme down. When that pathway is blocked, larotrectinib isn't cleared as quickly, so more of it can build up in your blood. Larotrectinib is an active drug (not a prodrug), so higher levels mean a stronger effect and a greater chance of side effects like dizziness, tiredness, nausea, or changes in your liver blood tests.

This isn't a reason to panic, but it's worth a real conversation. CBD products also vary a lot in strength. Since larotrectinib is a cancer treatment, please loop in your oncologist or pharmacist before adding or continuing CBD so they can watch you closely or adjust if needed.

What Is Larotrectinib Used For?

Brand name: Vitrakvi

Vitrakvi is used to treat adult and pediatric patients with solid tumors that have a neurotrophic receptor tyrosine kinase (NTRK) gene fusion — a specific genetic alteration that can drive tumor growth. It is used when the tumor is metastatic (has spread) or when surgery to remove it would likely cause serious harm, and when there are no satisfactory alternative treatments or the cancer has continued to grow after prior treatment.

Patients must be selected for Vitrakvi therapy based on an FDA-approved test that confirms the NTRK gene fusion in their tumor. This approval is under the FDA's accelerated approval pathway, based on how many patients responded to treatment and how long those responses lasted. Continued approval may depend on results from follow-up clinical trials.
