CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES

Lesinurad, Allopurinol falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Cannabidiol (cbd) interacts with.
In vitro and animal research shows that cannabidiol inhibits CYP2C9. In human studies, cannabidiol has been associated with an increase in plasma levels of topiramate and losartan, CYP2C9 substrates. Concomitant use of cannabidiol with CYP2C9 substrates may increase the risk for adverse effects from these substrates.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Duzallo combines two medicines, and the one to focus on here is lesinurad, which your liver clears partly through an enzyme called CYP2C9. CBD can slow that enzyme down. In good-quality studies, CBD raised the blood levels of other CYP2C9 medicines like losartan and topiramate, so it's reasonable to think lesinurad levels could climb a bit too.
Higher lesinurad exposure can mean more strain on the kidneys, which is the main safety concern with this drug, plus possible headache or reflux. Allopurinol isn't cleared this way, so it's mostly a lesinurad issue. None of this means you must avoid CBD, but please check with your pharmacist or doctor first, especially about staying well hydrated and keeping up with kidney lab tests.

CYTOCHROME P450 3A4 (CYP3A4) INDUCERS

Cannabidiol is a substrate of CYP3A4 enzymes. Theoretically, drugs that induce CYP3A4 enzymes might reduce the levels and effects of cannabidiol.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the key thing to know: in this pairing, it's your CBD that may be affected, not the gout medicine. The lesinurad portion of Duzallo can speed up a liver enzyme called CYP3A4, and that same enzyme helps break down CBD. So in theory, taking them together could clear the CBD out a bit faster and leave you with less CBD effect than you expected.
I want to be honest about the strength of this: it's a theoretical concern based on how these compounds behave on paper, not something proven in real patients. Your allopurinol and lesinurad should keep working normally for lowering uric acid. If you rely on CBD for a specific reason and feel it's not doing much, mention it to your pharmacist or doctor before changing anything.
