Mavacamten falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Cannabidiol (cbd) interacts with.

Cannabidiol may increase levels of drugs metabolized by CYP2C19.
Research shows that cannabidiol inhibits CYP2C19. In clinical studies and case reports, cannabidiol use resulted in significant increases in the serum levels of topiramate, methadone, citalopram, omeprazole, and N-desmethylclobazam, the primary active metabolite of clobazam. These chemicals are metabolized by CYP2C19. Concomitant use of cannabidiol with CYP2C19 substrates may increase the risk for adverse effects from these substrates.

Here's the thing to know about pairing CBD with Camzyos (mavacamten). Your liver uses an enzyme called CYP2C19 to help clear mavacamten out of your body. CBD slows that enzyme down, so the medicine can build up higher than expected. Mavacamten is a delicate drug that has to be dosed carefully, so even modest increases in its level matter.

If levels climb too high, mavacamten can weaken the heart's pumping too much, which may show up as shortness of breath, swelling, dizziness, or fatigue. This isn't a reason to panic, but it is a real, well-documented effect. Please don't change anything on your own. Talk with your cardiologist or pharmacist before adding CBD, since you may need extra echo monitoring.

In vitro and animal research shows that cannabidiol inhibits CYP3A4. In human studies and case reports, cannabidiol has been associated with an increase in plasma levels of the CYP3A4 substrates zonisamide, tacrolimus, everolimus, citalopram, midazolam, and methadone.

Here's the thing to know about pairing CBD with Camzyos (mavacamten): your body relies on a liver enzyme called CYP3A4 to help clear this medicine out. CBD can slow that enzyme down, which means mavacamten may build up higher than intended in your bloodstream.

Mavacamten works by gently weakening the heart's pumping force, so too much of it can push that effect too far. That could show up as a drop in your heart's pumping strength (ejection fraction), dizziness, shortness of breath, or swelling. Because mavacamten already requires careful dose monitoring and regular echocardiograms, even a modest bump in levels matters. Please don't start or stop anything on your own. Talk with your cardiologist or pharmacist first so they can guide you safely.
