Why this applies to Nilutamide

Nilutamide falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Cannabidiol (cbd) interacts with.

Cannabidiol may increase levels of drugs metabolized by CYP2C19.
Research shows that cannabidiol inhibits CYP2C19. In clinical studies and case reports, cannabidiol use resulted in significant increases in the serum levels of topiramate, methadone, citalopram, omeprazole, and N-desmethylclobazam, the primary active metabolite of clobazam. These chemicals are metabolized by CYP2C19. Concomitant use of cannabidiol with CYP2C19 substrates may increase the risk for adverse effects from these substrates.

Here's the simple version: your body uses a liver enzyme called CYP2C19 to help break down nilutamide. CBD slows that same enzyme down. So when you take both, nilutamide can clear out more slowly and build up in your blood a bit more than usual. Nilutamide is the active drug (not a prodrug that needs switching on), so more of it in your system means a stronger effect and a higher chance of its side effects.

For nilutamide, that could look like more nausea, trouble seeing in the dark or adjusting between light and dark, hot flashes, or feeling unusually tired. This interaction is rated moderate and is well supported, so it's worth a real conversation. Please don't start or stop anything on your own. Check with your pharmacist or oncologist first, since CBD product strengths vary a lot.

What Is Nilutamide Used For?

Brand names: Nilandron, Anandron

Nilutamide tablets are used together with surgical castration (removal of the testicles) to treat metastatic prostate cancer — prostate cancer that has spread to other parts of the body (Stage D2). It is not used alone; it works as part of this combined treatment approach.

For the best results, treatment with nilutamide should begin on the same day as surgical castration, or the day after.
