Here's a twist worth knowing: in this pairing, it's the CBD that may build up, not your osimertinib. CBD is partly broken down by a liver enzyme called CYP3A4, and Tagrisso can mildly block that enzyme. On paper, that could slow CBD's breakdown and leave more of it in your system, which may mean more of CBD's own effects like drowsiness, diarrhea, fatigue, or changes in liver enzymes.

I'll be honest, this one is theoretical, based on how the drugs behave rather than real-world reports, so I wouldn't lose sleep over it. But since osimertinib already affects the liver and is a serious cancer medicine, please loop in your oncologist or pharmacist before adding any CBD product.

In vitro and animal research shows that cannabidiol inhibits CYP3A4. In human studies and case reports, cannabidiol has been associated with an increase in plasma levels of the CYP3A4 substrates zonisamide, tacrolimus, everolimus, citalopram, midazolam, and methadone.

Here's the gist for Tagrisso (osimertinib): your liver uses an enzyme called CYP3A4 to help clear it from your body. CBD can slow that enzyme down. When the cleanup crew slows, more osimertinib can hang around, and that can nudge up its side effects.

With this drug, that might mean things like diarrhea, mouth sores, skin rash or dry skin, fatigue, or low blood counts. Rarely, osimertinib can affect the heart's rhythm (QT prolongation), so any added buildup is worth watching. The evidence here is solid for CBD blocking CYP3A4, though it hasn't been studied directly with osimertinib. Since this is cancer treatment, please don't start or stop anything on your own. Loop in your oncologist or pharmacist before adding CBD, and keep product quality in mind too.

About Osimertinib

What Is Osimertinib Used For?

Brand name: Tagrisso

Tagrisso is used to treat adults with non-small cell lung cancer (NSCLC) whose tumors have specific EGFR mutations — either an exon 19 deletion or an exon 21 L858R mutation — confirmed by an FDA-approved test. It is used in several settings: after surgery to reduce the risk of cancer returning (adjuvant therapy), for locally advanced stage III NSCLC that has not progressed after chemoradiation, and as a first-line treatment for locally advanced or metastatic NSCLC (including in combination with pemetrexed and platinum-based chemotherapy).

Tagrisso is also approved for adults with metastatic NSCLC that carries a specific resistance mutation called T790M, when the cancer has stopped responding to another EGFR-targeted therapy.
